Abstract

Operacijski zahvati na ramenu mogu se izvesti pomoću različitih anestezioloških tehnika koje uključuju opću anesteziju, regionalnu anesteziju i kombinaciju regionalne i opće anestezije. Provedene su mnoge studije koje su dokazale da je regionalna anestezija najbolja anesteziološka tehnika za operacijske zahvate na ramenu jer omogućuje najsigurnije izvođenje operacijskih zahvata te omogućuje najbolju postoperacijsku analgeziju. Najprikladnija metoda regionalne anestezije za postupke u području ramena je interskalenski živčani blok. Naziv interskalenski blok odnosi se na tehniku anesteziranja brahijalnog pleksusa u području vrata između prednjeg i srednjeg skalenskog mišića. Ova metoda osigurava dostatan gubitak osjeta, kako za minimalno invazivne artroskopske tehnike, tako i za otvorene operacije ramena. Uz primjerenu tehniku izvođenja bloka pomoću ultrazvuka i/ili perifernog živčanog stimulatora blok je povezan s relativno niskom stopom komplikacija. Operacije ramena povezane su s teškom postoperativnom boli te bi uvijek trebale biti popraćene odgovarajućom postoperativnom analgezijom, uključujući multimodalne postoperativne analgetičke pristupe. Primjena interskalenskog bloka omogućuje najbolju postoperacijsku analgeziju uz najmanje mogućih komplikacija. Regionalna anestezija udružena sa prikladnom postoperativnom analgezijom skraćuje vrijeme hospitalizacije bolesnika i ubrzava sam oporavak bolesnika te značajno pridonosi sigurnosti bolesnika i uspjehu operacijskog liječenja.

Abstract (english)

Shoulder surgery can be performed with different anesthetic techniques, including general anesthesia, regional anesthesia, and a combination of regional and general anesthesia. A number of studies have shown numerous benefits of regional anesthesia as the safest anesthetic option for shoulder surgery which provides the best postoperative analgesia. The most appropriate method of regional anesthesia for procedures in the shoulder area is interscalene nerve block. Interscalene nerve block refers to the technique of anesthetizing the brachial plexus in the neck between anterior and middle scalene muscle. This method provides sufficient loss of sensation for both minimally invasive arthroscopic techniques and open surgery. With appropriate administration of the technique, the block is associated with a relatively low complication rate. Shoulder surgery is associated with severe postoperative pain and should always be followed with appropriate postoperative analgesia, including multimodal postoperative analgesic approaches. Interscalene block provides excellent postoperative analgesia with the lowest complication rate. Regional anesthesia coupled with suitable postoperative analgesia provides shorter hospitalization, speeds up recovery and improves patient’s safety thus significantly increasing chances for successful surgical treatment.
