Abstract

Kronični rinosinuitis široko je rasprostranjena bolest različite simptomatologije. KRS definiramo kao upalu sluznice nosa i paranazalnih sinusa u trajanju od 12 tjedana, praćenu nazalnom opstrukcijom i/ili kongestijom te dodatnim simptomima kao što su bol i/ili pritisak u licu te smanjen osjet njuha. Usprkos širokoj rasprostranjenosti bolesti, dijagnostika te terapija KRS-a još uvijek nisu adekvatno razvijene, stoga velik broj pacijenata ostaje krivo dijagnosticiran. U ovom radu sudjelovalo je 150 pacijenata koji su prema EPOS smjernicama odgovarali dijagnozi KRS-a bez nazalnih polipa. Svaki pacijent podvrgnut je CT snimanju paranazalnih sinusa i svaka snimka ocijenjena je po Lund-Mackayu sustavu bodovanja. Nadalje, pacijenti su ispunili SNOT-22 upitnike i VAS koja je ispitivala teškoću njihovih simptoma. Cilj ovog rada bio je pronaći povezanost između stupnja upale sluznice te kliničkih simptoma koje je pacijent naveo. Rezultati su pokazali blagu pozitivnu povezanost između ispunjenosti prednjeg etmoidnog sinusa i osjeta boli te vrlo blagu negativnu povezanost između ispunjenosti prednjeg etmoidnog sinusa i osjeta njuha. Također je pokazano da obliteracija OMC dovodi do jače izraženosti nosnih simptoma. Nadalje, rezultati su pokazali da je simptom kašlja više izražen u osoba s obostrano izraženom bolesti. Ipak, navedene korelacije su vrlo blage te klinički nisu značajne stoga ne možemo govoriti o tome da raspodjela upale po sinusima značajno utječe na pojavu karakterističnih simptoma u kroničnom rinosinuitisu.

Abstract (english)

Chronic rhinosinusitis is one of the most prevalent diseases in the world. Chronic rhinosinusitis refers to a condition that lasts at least 12 weeks, despite being treated, and causes at least two of the following symptoms: nasal congestion. mucus discharge from the nose or mucus that drips down the back of the throat, facial pain, pressure, or "fullness". There are several factors which can increase the risk, or worsen the symptomes of CRS. These include: allergies, tobaco soke immune system disorders, infections etc. In this study, a series of patients with a clinical diagnosis of chronic rhinosinusitis underwent CT scans of the paranasal sinuses and each of the scans was staged using the Lund-MacKay system. In addition, the patients also filled the sino- nasal outcome test (SNOT-22) and, using the visual analog scale (VAS), they evaluated the pain they are experiencing daily. The object of this study was to find a correlation between the objective findings of the CT scan and the subjective symptoms reported by the patients. The results showed a weak positive correlation between the mucosal hyperplasion of the anterior ethmoid sinus and pain sensations and a very weak negative correlation between the mucosal hyperplasion of the anterior ethmoid sinus and the sensation of smell. It has also been shown that obliteration of OMC leads to increased expression of nasal symptoms, such as nasal congestion and nasal drainage. Furthermore, the results showed that the symptom of cough is more pronounced in people with bilateral disease. However, these correlations are very mild and clinically not significant so we can not say that the distribution of inflammation of the sinuses significantly affects the appearance of characteristic symptoms in chronic rhinosinusitis.
