Abstract Bronhoskopija je endoskopska metoda koja se primjenjuje u dijagnostici i terapiji bolesti traheobronhalnog sustava. Izvodi se u lokalnoj anesteziji u svim fazama liječenja plućnih bolesnika. Bolesnika treba fizički i psihički pripremiti za postupak, upoznati s ciljem, načinom i tijekom same pretrage, te ujedno istaknuti da njihova suradnja znatno olakšava pretragu.
Medicinski dijagnostički i terapijski postupci, pregledi i zahvati često su kod ljudi izvor straha i anksioznosti. U psihologiji stres je zahtjev koji od organizma traži prilagođavanje, suočavanje ili podnošenje. Stres je stanje u kojem se pojedinac nalazi kad je suočen s problemima ili zahtjevima okoline kojima se ne uspijeva prilagoditi. Pojam stresa danas je u uporabi radi opisivanja osjećaja nelagode, straha i nezadovoljstva.
Cilj: Cilj ovog istraživanja je prikupiti informacije o emocionalnom funkcioniranju pacijenata prije, tijekom i nakon postupka bronhoskopije, prisutnosti stresa kod bolesnika i utjecaja na kvalitetu izvođenja postupka.
Ispitanici i metode: Podatci prikupljeni za diplomski rad dobiveni su u ,,Klinici za plućne bolesti Jordanovac“, KBC-a Zagreb u trajanju od tri mjeseca. U radu su prikazani rezultati anonimnog anketnog istraživanja provedenog na uzorku od 50 ispitanika, oba spola.
Rezultati: U istraživanju, analiza ankete pokazuje da je razina straha kategorizirana prema stupnjevima visoka (3=40%; 4=30%), prisustvo brige o podnošenju postupka i nelagodi (50; 100%) te psihičko opterećenje dan prije postupka (37; 74%) prisutni. Tijekom postupka dolazi do porasta pulsa, pada saturacije u krvi i porasta krvnog tlaka koji se brzo oporavljaju. Nakon dijagnostičkog postupka rezultati pokazuju da je postupak bolesnicima bio stresan (27; 54%,) , osjećaju umor (31; 62%), a najviše su se bojali prolaza instrumenta kroz grlo (30; 60%). Rezultati ankete pokazuju da je mali postotak bolesnika odustao od postupka (3; 6%) i
kolabirao tijekom ili nakon postupka (4; 8%). Bolesnici smatraju korisnijim usmeno informiranje o dijagnostičkom postupku (46; 92%).
Zaključak: Strah je razlog teškog podnošenja dijagnostičkog postupka bronhoskopije. Daljnji rad treba usmjeriti na razgovor s bolesnicima, detaljnije objasniti tijek postupka, učiti vježbe disanja te pratiti neverbalnu komunikaciju u razgovoru s bolesnicima.
Abstract (english) Bronchoscopy is an endoscopic method that is applied in the diagnosis and therapy of diseases of the tracheobronchial system. It is performed under local anesthesia in all phases of treatment of pulmonary patients. The patient should be physically and mentally prepared for the procedures, acquaint with the purpose, the method and throughout of the procedure and also accentuate that his cooperation considerably eases procedure.
Medical diagnostic and therapeutic procedures, examinations and interventions are often a source of fear and anxiety in humans. In psychology, the definition of stress shows that the stress demand which of the organism requires adapting, coping or submission. Stress is a condition in which an individual is when faced with problems or demands of the environment that fail to adapt. The concept of stress today is used to describe the feelings of uneasiness, fear and frustration.
Objective: The aim of this study is to collect information about the emotional functioning of patients before, during and after the bronchoscopy procedure, the presence of stress in patients and the effect on the quality of the procedure.
Subject and Methods: The data collected for the graduate thesis were obtained in the "Clinical Center for Lung Diseases Jordanovac", KBC Zagreb for three months. The results are shown of an anonymous survey conducted on a sample of 50 subjects, both sexes.
Results: In the study, the analysis of the survey showed that the level of fear was high (3 = 40%; 4 = 30%), the presence of worries about filing the proceedings and discomfort (50; 100%) and the psychological load before the day (37; 74%) present. During the procedures, pulse and blood pressure growth, blood saturation is falling and rapidly recovering. After the diagnostic procedure, the results show that the procedure for the patients was sore (27, 54%), they felt fatigue (31; 62%) and the most feared was the passage of the instrument through the throat (30; 60%). Research results show that a small percentage of patients dropped out of the
procedure (3; 6%) and collapsed during or after the procedure (4; 8%). Patients find it more useful to have oral information about the diagnostic procedure (46; 92%).
Conclusion: Fear is the reason for the heavy submission of the diagnostic procedure of bronchoscopy. Further work should focus on conversation with the patients, more detaily verbally explain the principe of the procedure, learn the breathing exercises, and monitor nonverbal communication in conversation with the patients.
