Abstract

Integrirana skrb je koncept koji okuplja ulaze (input), izlaze (output), upravljanje i organizaciju usluga vezanih uz dijagnozu, liječenje, rehabilitaciju i promicanje zdravlja. Integracija skrbi dijeli se na nekoliko vrsta, razina i tipova. Koja je integracija skrbi potrebna ovisi o specifičnim uvjetima bolesti odnosno stanja. Kronične bolesti su stanja koja mogu uzrokovati trajnu nesposobnost, a zahtijevaju specifičnu i posebnu skrb.
Zakon o zdravstvenoj zaštiti svojim aktima omogućava uvođenje i provođenje integracije skrbi za kronično bolesnu djecu i njihove roditelje. Trenutni problemi u upravljanju zdravstvom otežavaju integraciju skrbi. Broj djece s kroničnim bolestima je oko 1%, no ta brojka se svake godine povećava, a zdravstveni sustav se ne mijenja. Djeca s kroničnim bolestima zahtijevaju složenije oblike liječenja, potrebna im je kronična, a ne samo akutna skrb. Prema rezultatima dosadašnjih dostupnih istraživanja u Hrvatskoj, roditelji djece s kroničnim bolestima su nezadovoljni postojećim sustavom zdravstvene zaštite, socijalne i zdravstvene skrbi. Integracijom skrbi djeca s kroničnim bolestima i njihove obitelji dobivaju kontinuitet preventivnih i kurativnih usluga prema specifičnim potrebama u svakom trenutku i na različitim razinama zdravstvene zaštite. Integracija skrbi pridonosi dobrobiti djece s kroničnim bolestima i njihovim roditeljima. Kako bi integracija skrbi bila korisna zahtjeva suradnju svih uključenih.

Abstract (english)

Integrated care is a concept that gathers inputs, outputs, management and organization of services related to diagnosis, treatment, rehabilitation and health promotion. Integration care is divided into several kinds, levels and types. Which type of integration care is needed depends on the specific characteristic of the disease or condition. Chronic illnesses are conditions that can cause permanent disability. Therefore, they require specific and special care.
The Act on Health Care allows introducing and implementing the integration of care for chronically ill children and their parents. Current health management issues make integrating care more difficult. The number of children with chronic diseases is about 1% and increasing each year, while the health system remains the same.
Children with chronic diseases require treatments that are more complex. They need chronic care, not just acute care. According to available data, parents of these children are dissatisfied with the existing health care system and also with social and health care. Through integration of care, children with chronic illnesses and their families receive continuity of preventative and curative services according to specific needs at any moment and at different level of health care. Integration of care contributes to the well-being of children with chronic illnesses and their parents. In order to make integration of care useful, it demands cooperation of all involved parties.
