Abstract

3 su glavna cilja u strategiji liječenja fibrilacije atrija, 1) kontrola ritma - postizanje i održavanje sinusnog ritma, 2) kontrola frekvencije - regulacija broja otkucaja, 3) prevencija tromboembolijskih incidenata. Kako bi se ostvarili ti ciljevi koriste se farmakoterapija, elektrokonverzija te invazivno liječenje. Osnovu invazivnog liječenja čini kateterska ablacija. Katetersku ablaciju prema metodi izvođenja možemo podijeliti na radiofrekventnu ablaciju i krioablaciju. Učinak kateterskom ablacijom se postiže električnom izolacijom pulmonalnih vena od lijevog atrija (za potpuni učinak potrebna je potpuna izolacija) i dodatnom ablacijom u stražnjem zidu lijevog atrija. Kateterska ablacija, učinjena u visokokvalitetnom centru, učinkovitija je od antiaritmičke terapije u održavanju sinus ritma. Stopa komplikacija, iako ne neznatna, podjednaka je kod kateterske ablacije i antiaritmičke terapije. Vaskularne komplikacije su najčešće komplikacije povezane s kateterskom ablacijom, a najčešće životno ugrožavajuće su tamponada srca i atrijskoezofagealna fistula dok je kod krioablacije vrlo učestala paraliza freničnog živca. Bolju učinkovitost kateterska ablacija pokazuje kod paroksizmalne fibrilacije atrija u odnosu na perzistentnu i permanentnu. Usporedba krioablacije i radiofrekventne ablacije pokazuje smanjenu stopu recidiva fibrilacije atrija te je smanjen broj rehospitalizacija kod krioablacije. Indikacija za kirurško liječenje fibrilacije atrija je dodatna srčana patologija uz fibrilaciju atrija.

Abstract (english)

There are 3 main goals in treatment of the atrial fibrillation, 1) rhythm control – achieviement and maintenance of the sinus rhythm, 2) frequency control – regulation of the number of heartbeats, 3) prevention of the thromboembolic incidents. In order to achieve these goals we use pharmacotherapy, electrical cardioversion and invasive treatment. The basis of invasive treatment is catheter ablation. Catheter ablation can be divided, according to the methods and tehnologies, into the radiofrequency ablation and cryoablation. Effect of the catheter ablation is primarily achieved through isolation of the pulmonary veins, probably requiring complete isolation for full effectiveness, and additional ablation in the posterior left atrial wall. Atrial fibrillation ablation, when performed in experienced centres by adequately trained teams, is more effective than antiarrhythmic drug therapy in maintaining sinus rhythm, and the complication rate, though not negligible, is similar to the complication rate for antiarrhythmic drugs. The most common complications of the catheter ablation of atrial fibrillation are vascular complications and the most common life-threatening complications are heart tamponade and atrioesophageal fistula while cryoablation is most commonly associated with the phrenic nerve palsy. Better effectiveness catheter ablation shows with treatment of paroxysmal atrial fibrillation than with persistent and permanent atrial fibrillation. Comparison of cryoablation and radiofrequency ablation shows reduced recurrence rate of atrial fibrillation and reduced number of rehospitalizations with cryoablation. Indication for surgical treatment of atrial fibrillation is additional cardiac pathology along atrial fibrillation.
