Abstract

U proteklih nekoliko desetljeća učinjeni su veliki napretci u razumijevanju imunosnog sustava i patogeneze raznovrsnih bolesti. Za široki spektar bolesti ustanovljena je imunološka podloga koja pridonosi nastajanju takvih stanja. Upravo je središnja uloga specifičnih imunosnih puteva u nastajanju određenih bolesti dovela do razvoja ciljane terapije. Svrha takve terapije je smanjiti opseg nuspojava koje vežemo uz standardnu terapiju određene bolesti, ali i omogućiti nove terapijske opcije za bolesnike kod kojih se konvencionalna terapija nije pokazala učinkovitom. Lijekovi koji se koriste kao biološka terapija napravljeni su u svrhu ciljanog djelovanja na određenu grupu gena ili proteina za koje se smatra da sudjeluju u patogenezi bolesti. Visoka učinkovitost biološke terapije dovela je do povećane i proširene upotrebe ovih lijekova u različitim bolestima. Istovremeno, sa sve širom primjenom biološke terapije u liječenju, opaža se sve više popratnih pojava ove terapije. Uočeno je da je koža jedan od organa gdje se vrlo često javljaju različite popratne pojave te će one biti opisane u ovom preglednom radu. Odluka kada primijeniti određeni biološki lijek nije uvijek jednostavna. U većini slučajeva, korist od primjene biološke terapije nadvladava moguću štetu od nastajanja nuspojava, ali određeni slučajevi ipak zahtijevaju prekid primjene biološke terapije. S obzirom na navedeno, i liječnici i pacijenti moraju dobro poznavati najčešće popratne pojave biološke terapije kako bi ih na vrijeme mogli prepoznati i liječiti.

Abstract (english)

Over the past few decades, major advances have been made in the understanding of the immune system and disease pathogenesis. An underlying immune pathogenesis has been identified for a wide spectrum of diseases. Those in which specific immune components have a central role lend themselves to targeted therapy. The aim of such treatment is to limit the complications of conventional treatment and provide options for otherwise treatment ‐ refractory disease. The agents used for biological therapy are designed to specifically target group of genes or proteins thought to be involved in the disease. In the recent years, the efficacy shown by biological therapy has led to its increased and extended use in different diseases. What is perhaps more important, as this overview will show, is the appearence of various adverese effects asssociated with their administration. Numerous cases of various skin lesions induced by these drugs have been reported with an increased incidence, highlighting the importance of the skin as a major target of the side effects of these drugs. Having that in mind, the decision of when to use such treatment in the clinic is not always straightforward. The therapeutic advantages of biological therapy, in most cases, exceed the risk of developing side effects, but some cases require the withdrawal of the drug. Doctors and patients should be aware of these potential adverse effects for an early and correct diagnosis and appropriate treatment.
