Abstract

Masovne nesreće, poput potresa, predstavljaju bitan uzrok mortaliteta u današnje vrijeme. Za uspješnost odgovora bitna je koordinacija svih sudionika, od izvanbolničke hitne pomoći do bolničkog sustava. Kod ovakvog ovakvog tip katastrofa brz i efikasan odgovor te usklađenosti djelovanja na svim razinama je presudan.
Termin „trijaža” potječe od francuske riječi trier, koja znači odrediti, izabrati. Počeci trijaže prvi put se spominju u Drugom svjetskom ratu kod francuskih liječnika koji su pružali pomoć ozlijeđenima na bojnom polju. Prva sustavna primjena trijaže započela je u Australiji 1993. godine. Danas postoji više modela provođenja trijaže, među njima Australsko-azijska trijažna ljestvica (ATS), Kanadska trijažna ljestvica procjene (CATS), Mančesterska trijažna ljestvica (MTS) i Emergency Severity Index (ESI) koje su imale najveći utjecaj na razvoj procesa trijaže.
Budući smo danas svjedoci sve češćih seizmoloških aktivnosti i razornih potresa koji se događaju diljem svijeta, potreba za brzim i odgovarajućim postupanjem s ozlijeđenima nameće se sve više. Prema podacima Državne uprave za zaštitu i spašavanje Republike Hrvatske (DUZS) iz 2011. godine i lokaciji grada Zagreba postoji vrlo velika vjerojatnost za snažan potres. Pripremljenost svih sustava za takvu katastrofu uključuje i protokole kojima se definira na koji način i kroz koje službe djelovanja zbrinuti unesrećene na terenu pogođenim potresom. Trijaža je početak samog postupka, a kako će se ona provesti, prema kojim kriterijima, te tko će imati prednost u zbrinjavanju ozljeda nastalih potresom, predmet je ovoga rada. Također, u radu su prikazane metode koje se koriste, te protokoli i smjernice za postupanje prilikom razornih potresa.

Abstract (english)

Mass casualties, such as earthquakes, are an important cause of mortality at present days. For successful response, it is important to coordinate all participants, from out-of-hospital emergency to hospital system. For this type of disaster the need for a fast and efficient response and compliance of action at all levels is crucial.
The term "triage" is derived from the French word (trier), which means to determine, to choose. The beginnings of triage were first mentioned in World War II by French doctors who provided help to the injured in the battlefield. The first systematic triage application started in Australia in 1993. There are more triage protocols today, including the Australian-Asian Triage Scale (ATS), the Canadian Triage Scale Assessment (CATS), the Manchester’s Trials Scale (MTS) and Emergency Severity Index (ESI) that have had the greatest impact on the development of triage processes.
We are witnessing more frequent seismological activities and devastating earthquakes occurring all over the world. According to the data available from Croatian state agency for protection and rescue, and location of the city of Zagreb, there is a very high probability for a severe earthquake. Preparedness of all services for that kind of disaster include protocols which will define how and which services will take care of injured at the field affected by the earthquake. How will the triage be conducted, according to which criteria, and who will have priority for care is the main topic of this paper. Likewise, methods, protocols and guidelines specific for earthquake management are presented.
