| Abstract | Neplodnost je stanje u kojem par reproduktivne dobi ne uspijeva zanijeti u toku godine dana uz redovne, nezaštićene spolne odnose. Nakon tog vremenskog perioda, trebalo bi započeti s obradom neplodnog para, a kod žena starijih od 35 godina s obradom bi trebalo početi nakon 6 mjeseci neuspješnog pokušavanja. Incidencija neplodnosti u populaciji je oko 15%. Uzroci neplodnosti su različiti, a mogu se podijeliti na ženske i muške uzroke te na idiopatsku neplodnost. Etiologija ženske neplodnosti može biti vezana uz poremećaje ovulacije i uz poremećaje na putu oplodnje, transporta i implantacije. Muški uzroci neplodnosti mogu se podijeliti na pregerminalne, germinalne i postgerminalne uzroke. U obradi neplodnog para valja se voditi principom postupnosti. Prvo treba provesti jednostavnije i neagresivne pretrage, a kasnije složenije i agresivnije. Osnova obrade su anamneza i fizikalni pregled, kod žena popraćeni ginekološkim pregledom s uzimanjem citološkog obriska. Nadalje, kod žena je potrebno dokazati ovulaciju. Neke od metoda koje se koriste za tu svrhu su trakice koje mjere koncentraciju LH u urinu, mjerenje bazalne tjelesne temperature, određivanje koncentracije progesterona u krvi te ultrazvučni pregled. Ako se utvrdi ovulatorna disfunkcija, potrebna je daljnja obrada kako bi se otkrio njen uzrok. Najčešći uzroci oligoovulacija su hormonalni te je zato potrebno izmjeriti koncentracije FSH, LH, E2, PRL, TSH i androgena u krvi. Ovarijska rezerva označava broj preostalih jajnih stanica žene, a moguće ju je procijeniti mjerenjem razina FSH i AMH u krvi te ultrazvučnim mjerenjem broja antralnih folikula. U obradi žene, potrebno je još procijeniti stanje jajovoda i maternice. Za to se koriste slikovne metode, HSG, HyCoSy, UZV, SIS, te endoskopske metode, laparoskopija i histeroskopija. Osnovna pretraga u obradi muške neplodnosti analiza je ejakulata, kojom se gleda broj, pokretljivost i izgled spermija. Prvi korak liječenja dijagnosticiranje je uzroka. Ukoliko se uzrok pronađe, liječenje je usmjereno njegovom rješavanju. Ukoliko se obradom ne utvrdi jasan uzrok neplodnosti ili jednostavnije metode liječenja neplodnosti ne daju rezultat, tada se koriste metode MPO-a koje uključuju IUI, IVF i ICSI. Svakom postupku MPO-a trebala bi prethoditi kvalitetna obrada jer odabir postupka liječenja i vrsta stimulacije, a samim time i uspješnost postupka, ovise o podacima prikupljenim obradom. |
| Abstract (english) | Infertility is defined as the failure to conceive after 12 months of regular unprotected intercourse. After this period of time, evaluation of infertility should begin, while in women older than 35 years, evaluation should begin after 6 months of an unsuccessful attempt. Incidence of infertility in the population is about 15%. The causes of infertility are various and can be divided into female and male causes and idiopathic infertility. Etiology of female infertility may be related to ovarian disorders and disorders related to fertilization, transport and implantation. Male causes of infertility can be divided into pregerminal, germinal and postgerminal causes. In the evaluation of the infertile couple, simple and non-invasive procedures should be done first. The basic evaluation includes a careful history and physical examination of both partners, in women accompanied by a gynecologic examination with cytological smear. Furthermore, ovulation is detected in women. Some of the methods used for this purpose are urinary LH kits, basal body temperature measurement, midluteal progesterone level and ultrasound examination. If ovulatory dysfunction is found, further processing is required to detect its cause. It is reasonable to undertake an endocrine evaluation which includes the determination of the levels of LH, FSH, TSH, E2, prolactin and androgens. The ovarian reserve describes the number of ovarian cells remaining and can be estimated by measuring the levels of FSH and AMH in the blood and by antral follicle count. Imaging methods, such as ultrasound, HSG, HyCoSy, and SIS are used to assess the condition of the fallopian tubes and the uterus. Also, endoscopic methods, laparoscopy and hysteroscopy can be used. Semen analysis is the cornerstone of male infertility evaluation. It provides information on the number, appearance and sperm mobility. The first step of treatment is to diagnose the cause. If the cause is found, the treatment is directed to its resolution. If the evaluation does not establish a clear cause of infertility or treatment does not produce results, then methodes of assisted reproduction are used. They include IUI, IVF and ICSI. Quality evaluation should precede any assisted reproduction procedure because the choice of the treatment procedure and the type of procedure, and thus the success of the procedure, depend on the data collected by the evaluation. |
