Abstract

Moždani udar (MU) je drugi uzrok smrti u svijetu i prvi uzrok invalidnosti. Ishemijski MU je značajno češći (oko 80%) u odnosu na hemoragijski MU (oko 20%). Najčešći mehanizam nastanka ishemijskog MU je tromboembolijski. Ovisno o indikaciji, rekanalizacijsko liječenje je najučinkovitiji način liječenja ishemijskog moždanog udara. Sistemska tromboliza intravenskom aplikacijom rekombinatnog tkivnog aktivatora plazminogena (rtPA) je najčešće primjenjivana rekanalizacijska metoda u svijetu (2 do 10% svih bolesnika sa IMU). Intracerebralno krvarenje je najteža komplikacija liječenja bolesnika sistemskom trombolizom, ali najveći broj krvarenja je ipak asimptomatski. Kako bi primjena sistemske trombolize bila što uspješnija, ali i što sigurnija postoji niz indikacija, kontraidikacija i relativnih kontraindikacija propisanih međunarodnim smjernicama. Nedostatak liječenja rTPA-om je ograničen vremenski prozor primjene od 3,5 odnosno 4,5 sata od početka simptoma ishemijskog MU. Mehanička trombektomija je rekanalizacijska metoda koja je 2015. godine uvedena u smjernice za liječenje ishemijskog MU unutar 6 sati od nastanka simptoma. Kao i kod sistemske trombolize moguće su komplikacije od kojih je najteža intracerebralna hemoragija. Kako bi mehanička trombektomija bila što sigurnija i učinkovitija međunarodnim je smjernicama propisan niz indikacija, kontraindikacija i relativnih kontraindikacija. Jedna od pozitivnih strana mehaničke trombektomije jest proširenje terapijskog prozora na čak 24 sata u točno indiciranih bolesnika čime je otvorena mogućnost liječenja većeg broja bolesnika. Premda je rekanalizacijsko liječenje donijelo revoluciju u liječenje ishemijskog moždanog udara i značajno smanjilo smrtnost i onesposobljenost, još uvijek postoji veliki prostor za napredak. Napredak se očekuje u širenju indikacija, tehnikama izvođenja i boljoj organizaciji zdravstvenog sustava kako bi se još dodatno smanjila smrtnost i onesposobljenost te kako bi se povećala dostupnost ove vrste liječenja u svih bolesnika u kojih je takvo liječenje indicirano.

Abstract (english)

Stroke is the second leading cause of death in the world and the first cause of disability. Ischemic stroke is significantly more frequent (about 80%) compared to hemorrhagic stroke (about 20%). The most common mechanism of ischemic stroke development is thromboembolic. Depending on the indication, recanalisation treatment is the most effective method of treating ischemic stroke. Systemic thrombolysis by intravenous application of recombinant tissue plasminogen activator (rTPA) is the most commonly used recanalisation method in the world (2 to 10% of all patients with ischemic stroke). Intracerebral hemorrhage is the most severe complication of treatment of patients with systemic thrombolysis, but most of the cases are asimptomatic. In order to make systemic thrombolysis more successful, but as safe as possible, there are a number of indications, contractions and relative contraindications prescribed by international guidelines. The downside of rTPA treatment is the limited time window for application which is 3.5 and 4.5 hours since the onset of ischemic stroke symptoms. Mechanical thrombectomy is a recanalization method that was introduced in the guidelines in 2015. and is used for the treatment of ischemic stroke within 6 hours of the onset of symptoms. As with systemic thrombolysis, complications are possible, of which the most severe is intracerebral hemorrhage. In order to make the mechanical thrombectomy safer and more effective, a range of indications, contraindications and relative contraindications have been provided by international guidelines. One of the positive aspects of mechanical thrombectomy is the extension of the therapeutic window for up to 24 hours in exactly-indicated patients, thus opening up the possibility of treating a large number of patients. Although the recanalization treatment has brought about a revolution in the treatment of ischemic stroke and has significantly reduced mortality and disability, there is still room for improvement. Progress is expected in expansion of indications, techniques of performing and better organization of the health system to further reduce mortality and disability and to increase the availability of this type of treatment in all patients whose treatment is indicated.
