Abstract

Uvod i cilj: Učestalost metaboličkoga sindroma razlikuje se s obzirom na dob, spol, stupanj
uhranjenosti i primijenjene kriterije, te se kreće od 0-11 % u općoj populaciji, a u populaciji
prekomjerno teške i pretile djece od 5-33%. Cilj ovoga rada je bio utvrditi učestalost
metaboličkoga sindroma među adolescentima u dobi od 17 do 19 godina u Hrvatskoj. -----
Ispitanici i metode: Ispitanici u istraživanju bili su 50%-tni proporcionalni uzorak inicijalne
kohorte iz reprezentativnoga uzorka učenika prvih razreda osnovnih škola u Hrvatskoj,
školske godine 2003./2004. Provedena je antropometrija tjelesne visine, tjelesne mase,
arterijskoga tlaka i pulsa, opsega struka, izračunatog indeksa tjelesne mase te laboratorijska
analiza krvi za glukozu u serumu, lipoproteine visoke gustoće i trigliceride. Za postavljanje
dijagnoze metaboličkoga sindroma korišteni su International Diabetes Federation kriteriji za
djecu stariju od 16 godina. Podaci su analizirani metodom deskriptivne statistike. -----
Rezultati: Analizom rezultata istraživanja obuhvaćeno je 447 (92,16%) od 485 ispitanika
planiranih u uzorku. Utvrđena je učestalost metaboličkoga sindroma u adolescenata u dobi od
17 do 19 godina od 2,5%. S obzirom na uhranjenost, najveća učestalost metaboličkoga
sindroma utvrđena je u skupini pretilih gdje je metabolički sindrom imao svaki treći ispitanik,
osam od dvadesetpetero ispitanika. U skupini prekomjerno teških metabolički sindrom je
utvrđen kod troje od šezdesetsedmero ispitanika, dok u skupini normalno uhranjenih i
pothranjenih metabolički sindrom nije utvrđen. -----
Zaključak: Hipoteza da se učestalost metaboličkoga sindroma među adolescentima u
Hrvatskoj ne razlikuje od učestalosti u populaciji adolescenata u drugim zemljama potvrđena
je. Koncept metaboličkoga sindroma u populaciji djece i mladih važan je jer identificira
multipli rizični fenotip u toj populaciji. Kroz rad Službe za školsku medicinu, provedbom
Programa mjera specifične zdravstvene zaštite školske djece i mladih moguće je otkriti djecu
s prisutnim čimbenicima rizika i uputiti ih dalje na laboratorijske pretrage u svrhu ranog
otkrivanja metaboličkoga sindroma.

Abstract (english)

Background and Aim: Prevalence of metabolic syndrome among children and youth ranges
from 0 to 11% among general population and from 5 to 33% among overweight and obese
children. The aim of the study was to determine prevalence of metabolic syndrome in
adolescents aged 17 to 19 years in Croatia. -----
Respondents and methodology: subject in the study were 50% proportional sample of the
initial cohort from a representative sample of first-grade students of elementary school year
2003/2004 in Croatia. Data were obtained by anthropometry and laboratory blood testing.
International Diabetes Federation criteria were used to diagnose metabolic syndrome. Data
were analysed using descriptive statistics. -----
Results: research analysis included 447 respondents (223 boys). Prevalence of metabolic
syndrome among adolescents was 2.5%, and it was the highest in the group of obese
adolescents (eight of 25), in the group of overweight it was three of 67. In the group of normal
weight and underweight there was no determined metabolic syndrome. -----
Conclusion: The concept of metabolic syndrome in children and youth is important because
of identifying multiple risk phenotypes in this population. School Health Service could detect
children with risk factors and refer them to laboratory tests for early detection of metabolic
syndrome.
