Abstract Dugotrajne i složene ortopedske operativne zahvate izmjene zglobova kuka, koljena i ramena te implantaciju endoproteze redovito prati jak i kontinuiran intenzitet akutne boli. Neodgovarajuće tretiranje boli može doprinijeti nizu nepoželjnih popratnih sistemskih učinaka na organizam. Nadalje, neadekvatno tretirana bol ujedno indirektno utječe i na zadovoljstvo pacijenta pružanjem zdravstvene skrbi. Navedeni nepovoljni učinci akutne boli mogu biti odgovorni za usporavanje oporavka, kao i onemogućavanje odgovarajućeg stupnja fizikalne terapije i rehabilitacije, što u konačnici dovodi do produženog boravka u bolnici. Stoga, intenzitet poslijeoperacijske boli značajno utječe na oporavak pacijenta. Uzevši u obzir navedene činjenice, procjena boli i postizanje odgovarajuće analgezije izrazito su bitni faktori, a da bi se postigao optimalan analgetski učinak, potrebno je uvažavati individualne razlike u subjektivnom doživljavanju intenziteta boli svakog pacijenta kod izlaganju identičnom operativnom zahvatu ili traumi. Prema tome, nameće se zaključak da postizanje adekvatne analgezije predstavlja veliki izazov, jer s jedne strane, bol je prije svega individualan osjet koji je specifičan za svakog pacijenta ponaosob, a s druge strane, zahtjeva interdisciplinarni pristup i suradnju više profila zdravstvenih djelatnika s pacijentom, a ovisno o specifičnosti pacijenta, ponekad zahtjeva uključivanje i sudjelovanje obitelji. Primjenom multimodalne analgezije s kombinacijom analgetika različite grupe i mjesta djelovanja, omogućeno je učinkovito smanjenje intenziteta boli, ali i razvoja potencijalnih nuspojava lijekova zbog mogućnosti primjene manje dnevne doze analgetika uslijed kombinacije farmakoloških metoda tretiranja boli. Uloga medicinske sestre, kao sudionika interdisciplinarnog tima, je kompleksna i najviše dolazi do izražaja u procjeni i evaluaciji boli, primjeni farmakoloških i nefarmakoloških metoda suzbijanja boli, uočavanju i tretiranju potencijalnih neželjenih pojava analgetske terapije i u edukaciji pacijenta.
Abstract (english) Lengthy and complex orthopaedic surgery changes of the hip, knee and shoulder joints, as an implantation of endoprothesis, regularly are accompanied by strong pain intensity. Inadequate pain treatment can contribute to a numerous undesirable systemic side effects in the body. Further, inadequately treated pain also indirectly influence on patient satisfaction about providing health care. The aforementioned adverse effects of acute pain may be responsible for slowing recovery as well as disabling appropriate level of physical therapy and rehabilitation, which ultimately leads to prolonged hospital stay. Therefore, the intensity of postoperative pain significantly affects on the patient's recovery. Given the above facts, achieve adequate analgesia is extremely essential factor, and to accomplish optimal analgesic effect, it is necessary to take into account individual differences in the subjective experience of the pain intensity in each patient with exposure to identical surgery or trauma. Thus, implies that the pain assessment and achieving an adequate analgesia are major challenges because, on the one hand, pain is primarily an individual feels that is specific to each patient, and the other, demands an interdisciplinary approach and the cooperation between different profiles of health care professionals with patient, and depending on the specifics of the patient, sometimes requires family inclusion and participation. Applying a multimodal analgesia with analgesics of different groups of actions, it is possible to effectively decrease pain and also a development of potential drug side effects due to the possibilities of applying smaller daily doses of painkillers by combination of pharmacological methods for pain treating. The role of nurse, as a participant of interdisciplinary team, is complex and the most prominent is in the assessment and evaluation of pain, application of pharmacological and non-pharmacological methods of pain management, in recognizing and treating potential adverse effects of analgesic therapy and in the patient´s education.
