Abstract

Šum u ušima jedan je od najstarijih medicinskih pojmova. Definiramo ga kao percepciju zvuka uz odsutnost vanjskog podražaja koji se najčešće opisuje kao zujanje, hučanje, siktanje, zvonjenje ili zviždanje, ali ponekada i kao kompleksniji zvuk, melodiju. Danas probleme sa simptomom šuma u ušima ima 10% do 15% ukupne populacije te je u do 0.5% bolesnika toliko izražen da ozbiljno utječe na kvalitetu života. Muški spol i etnička pripadnost bijeloj rasi nehispanskih korijena nepromjenjivi su faktori rizika, ali postoje brojni drugi faktori poput pušenja, dijabetesa melitusa, dislipidemije, hipertenzije, pretilosti i izloženosti buci na koje možemo, i trebamo utjecati jer lijeka za šum u ušima još uvijek nema. Kao potencijalni uzrok ovog neugodnog simptoma, u obzir se mogu uzeti mnoga stanja te je bitno imati ih na umu kada pristupamo bolesniku sa šumom u ušima kako bi što efikasnije došli do srži problema i započinjanja adekvatnog liječenja. Kako se i sam šum u ušima dijeli u dvije skupine, objektivni šum i subjektivni šum, tako i diferencijalnu dijagnozu dijelimo u dvije kategorije: diferencijalnu dijagnozu objektivnog i diferencijalnu dijagnozu subjektivnog šuma
u ušima. Cilj ovog diplomskog rada je kratak pregled najčešćih uzroka šuma u ušima.

Abstract (english)

Tinnitus is one of the oldest medical terms. It is defined as perception of sound in the absence of an external stimulus that is most often described as a buzzing, roaring, hissing, ringing, or whistling, but sometimes as a more complex sound, a melody. The prevalence of tinnitus ranges from 10% to 15%, and up to 0.5% is so pronounced that it seriously affects the quality of life. Nonmodifiable risk factors are male gender and non-Hispanic Whites' ethnicity. Numerous other factors like smoking, diabetes mellitus, dyslipidemia, hypertension, obesity, and noise can, and should be assessed and modified for there is still no cure for tinnitus. Many conditions can be considered a potential cause of this unpleasant symptom, and it is essential to keep in mind that we should get to the core of the problem as efficiently as possible and start an adequate treatment course. Tinnitus is divided into two groups, objective and subjective tinnitus, so differential diagnosis is divided into two categories as well: differential diagnosis of objective, and differential diagnosis of subjective tinnitus. The aim of this thesis is a brief overview of the most common causes of tinnitus.
