Abstract

Brojne benigne i maligne bolesti dojke se liječe kirurški. Kirurgijom dojke se obično bavi plastični ili onkološki kirurg. Među benigne bolesti dojke koje se liječe kirurški se ubrajaju kongenitalne anomalije, apscesi, patološki secernirajući kanalići i benigni
tumori. Svaki rak dojke I., II. i III. stadija indikacija je za kiruršku terapiju. Operacije dojke mogu biti jednostavne incizije za liječenje apscesa i ekscizije za liječenje tumora, secernirajućih kanalića i nekih kongenitalnih anomalija. Ovisno o stadiju bolesti, rak dojke se može operirati poštednim ili radikalnim ekscizijskim metodama. Poštedne metode uključuju tumorektomiju, kvadrantektomiju i segmentektomiju, a radikalne metode mastektomiju. Treća mogućnost između poštednog i radikalnog zahvata je onkoplastična operacija dojke. Među kompleksnije zahvate na dojci se ubrajaju rekonstruktivni zahvati nakon mastektomija i estetske operacije
kongenitalnih anomalija. S obzirom da je dojka fiziološki, psihološki i fizički bitan organ žene, treba uzeti u obzir i te aspekte pri kirurškom liječenju bolesti dojke. Kod većine operacija dojke vrlo je bitno da kirurg i pacijentica dogovorno odluče o operaciji te usklade pacijentičine želje i očekivanja s mogućnostima koje pruža današnja kirurgija dojke.

Abstract (english)

Numerous benign and malignant breast diseases can be surgically treated by breast surgeon. To become breast surgeon, general surgeon has to complete medical training in plastic surgery or surgical oncology. Some benign diseases are treated surgically, including congenital anomalies, abscesses, single duct nipple discharge and some benign tumours. Patients with stage I, II and III breast cancers are candidates for surgical therapy. Breast surgeries can be simple, such as incisions to treat abscesses or excisions used to remove some tumours, secreting lactiferous duct or some congenital anomalies. There are several types of breast cancer surgeries, including breast-conserving surgery and radical surgery. The type of the surgery depends on the stage of the cancer. Breast-conserving surgery refers to lumpectomy, quadrantectomy and segmentectomy, while radical methods refer to mastectomy. The third option, between breast-conserving surgery and mastectomy is oncoplastic surgery. More complex surgeries include breast reconstructions after mastectomies and aesthetic procedures to treat congenital anomalies. When it comes to breast surgery, it must be taken into account that breast is very important organ for a woman in physiological, psychological and physical aspect. It is very important for doctor and patient to decide on surgery together and to align the patient's wishes and expectations with the possibilities of today's breast surgery.
