Abstract

Ozljeda kralježnične moždine kompleksna je ozljeda s nizom komplikacija i posljedica
koje je teško liječiti i rehabilitirati. Godišnja incidencija OKM-a u svijetu iznosi između 10,4 i
83 slučaja na milijun stanovnika s velikim utjecajem na zdravstveni sustav. Najčešći uzroci koji
dovode do ozljede kralježnične moždine su automobilske nesreće, nasilje, padovi te sportske
ozljede. Ozljede kralježnične moždine, potpune ili nepotpune, dovode do invaliditeta koji
utječe na pacijenta, obitelj, zajednicu te može ostaviti katastrofalne posljedice, ne samo s
medicinske već i s emocionalne, društvene, ekonomske i psihološke strane. Specijaliziranim
rehabilitacijskim programima u kojima sudjeluje interdisciplinarni tim posljedice mogu biti
ublažene. Interdisciplinarni tim vođen je fizijatrom te uključuje obitelj pacijenta, fizioterapeute,
radne terapeute, medicinske sestre/tehničare, dijetetičare, psihologe, socijalne radnike i prema
potrebama pacijenta ostale specijaliste. Liječenje i rehabilitacija pacijenta s OKM-om dug je,
skup i iscrpljujući proces, obično traje 3-6 mjeseci, ovisno o razini i potpunosti ozljede kao i o
prisutnim komplikacijama. Rehabilitacijski proces u ranoj, akutnoj fazi važan je kako bi se
spriječile kontrakture i sačuvala mišićna snaga, spriječila osteoporoza te osigurala normalna
funkcija probavnog i respiratornog sustava. U kroničnoj fazi rehabilitacije najvažniji je cilj
omogućiti pacijentu samostalno kretanje u skladu s njegovom razinom ozljede, omogućiti
integraciju pacijenta u društvo te obnoviti psihološko i emocionalno stanje pacijenta.

Abstract (english)

Spinal cord injury (SCI) is a complex injury with a number of complications and
consequences that are difficult to treat and rehabilitate. Spinal cord injury (SCI) shows an
incidence of 10.4-83 cases/million/year globally and remains a significant source of morbidity
and cost to society. The primary cause of spinal cord injury is automobile collisions, followed
by violence, falls, and injuries in sporting events. The patient is most frequently a young male.
Whether complete or incomplete SCI lead to disability that affects the patient, family,
community and can leave catastrophic consequences, not only medically but also emotionally,
socially, economically and psychologically. With special rehabilitation programme in which an
interdisciplinary team participates those effects can be alleviated. The team is led by physiatrist
and consists of the patient´s family , physiotherapist, occupational therapist, nurses, dietician,
psychologist, social worker and other consultant specialists as necessary. The treatment and
rehabilitation period is long, expensive and exhausing in SCI, it may last 3-6 months, depending
of the level and completeness injury as well as complications. Early rehabilitation is important
to prevent joint contractures and the loss of muscle strength, conservation of bone density, and
to ensure normal functioning of the digestive and respiratoy system. In chronic rehabilitation
period the most important goal is realization of the independent mobilization and to ensure the
maximum independence related to the level of the patient`s injury , integration of the patient to
society and restore the patient`s psychological and emotional state.
