Abstract

Psorijatični artritis i ankilozantni spondilitis najčešći su entiteti skupine upalnih reumatskih bolesti – seronegativnih spondiloartropatija. U seronegativne spondiloartropatije spadaju i enteropatski artritis, reaktivni artritis te nediferencirani spondiloartritis. Razlikuju se od seropozitivnih reumatskih bolesti kao što je reumatoidni artritis po nedostatku protutijela i širokom spektru simptoma. Učestalost bolesti varira ovisno o populaciji, a veća se učestalost veže uz pojavnost humanog leukocitnog antigena HLA-B27 u genomu. U seronegativnim spondiloartropatijama otežana je uniformna primjena dijagnostičkih postupaka i liječenja. Uz tehnološki napredak u dijagnostici i novim saznanjima o mehanizmima nastanka bolesti stvorene su nove smjernice u liječenju. Primjena novijih lijekova sve se češće uvodi uz adekvatne indikacije i prije iscrpljenja terapijskih mogućnosti klasičnom terapijom. Inhibitori interleukina 17 (IL-17) dio su nove terapijske opcije u liječenju spondiloartropatskih bolesti. Spadaju u skupinu bioloških lijekova – imaju biološko podrijetlo i selektivno supresivno djeluju na imunološki sustav. Iako su nesteroidni protuupalni lijekovi i konvencionalni lijekovi koji utječu na tijek bolesti najčešći farmakološki izbor liječenja, u 21. stoljeću primjena biološke terapije dobiva na značaju i nalazi svoje mjesto u liječenju. Prikaz najčešćih spondiloartropatija, kompleksnost liječenja i uloga inhibitora interleukina 17 u terapiji su tema ovog rada.

Abstract (english)

Psoriatic arthritis and ankylosing spondylitis are the most common entities in a family of inflammatory rheumatologic diseases – seronegative spondyloarthropathies. Enteropathic arthritis, reactive arthritis and undifferentiated spondyloarthritis are also a part of seronegative spondyloarthropathies. They are different from other seropositive rheumatic diseases, such as rheumatoid arthritis, for their lack of antibodies and a broad spectrum of symptoms. Disease frequency varies in different population groups and is usually linked to human leukocyte antigen HLA-B27 presence in genome. Diagnosis and treatment cannot be performed uniformously in seronegative spondyloarthropathies. New treatment guidelines are formed due to advances in diagnostic technology and new knowledge on pathogenesis. Administration of newer drugs occurs more often using adequate indications even before classic therapy treatment becomes inefficient. Interleukin 17 (IL-17) inhibitors are a new treatment option for spondyloarthropathies. They are a part of biological drugs – their origin is biologic and they suppress immune system selectively. Although nonsteroidal anti-inflammatory and diesase-modifying anti-rheumatic drugs are the most often used pharmacological treatment, in the 21st Century biological drug administration finds its meaning as a treatment option. This paper is a review of the most common spondyloarthropathies, treatment complexity and interleukin 17 inhibitors possible treatment role.
