Abstract Incidencija infekcije humanim papiloma virusom (HPV) u stalnom je porastu, te se smatra najčešćom virusnom spolno prenosivom bolešću. Pretpostavka je kako će se 80% seksualno aktivnih muškaraca i žena u nekom trenutku u životu zaraziti HPV-om. U većini zapadnih zemalja incidencija HPV genitalnih infekcija najviša je u dobi od 15. do 25. godine, odnosno tijekom desetljeća nakon prvog spolnog odnosa, te značajno opada nakon 40. godine života. Poznato je više od 100 tipova HPV-a, od kojih otprilike 30 tipova ima onkogeni potencijal i povezani su s povećanim rizikom razvoja karcinoma. Većina HPV infekcija je samolimitirajuća, no visokorizični HPV genotipovi mogu perzistirati i uzrokovati karcinom. Niskorizični HPV genotipovi, od kojih su najpoznatiji tip 6 i tip 11, uzrokuju anogenitalne bradavice. Genitalne infekcije uzrokovane HPV-om, manifestiraju se različitim kliničkim entitetima, od šiljastih kondilomi-condilomata acuminata, ravnih kondiloma-condylomata plana, gigantski kondilom Buschke-Löwenstein, papulosis Bowenoides, kao i brojne kliničke manifestacije intraepitelnih lezija (IN) vanjskog genitalnog sustava. Liječenje genitalnih HPV infekcija predstavlja velik izazov, etiološka terapija HPV-a još uvijek ne postoji, a recidivi bolesti su česti. Postoji nekoliko terapija za topikalno liječenje HPV infekcije, od kojih ni jedna za sada nije zlatan standard. Budući da je terapija ponekad neugodna i bolna, a recidivi su česti, liječenje predstavlja izazov kako za pacijenta tako i za liječnika. S obzirom na sve navedeno, te na dostupnost cjepiva protiv HPV-a, logično je kako je cjepivo suvremena strategija za prevenciju HPV uzrokovanih bolesti.
Abstract (english) The incidence of human papilloma virus (HPV) infection is increasing, and it is considered to be the most common viral sexually transmitted disease. It is estimated that 80% of sexually active men and women will become infected with HPV at some point in their lives. In most Western countries, the incidence of HPV genital infections is highest between the ages of 15 and 25, or during the decade after the first sexual intercourse, and significantly decreases after the age of 40. More than a 100 types of HPV are known, of which approximately 30 types have an oncogenic potential and are associated with an increased risk of developing cancer. Most HPV infections are self-limiting. However, high-risk HPV genotypes can persist and eventually become cancerogenic. Low-risk HPV genotypes, among which type 6 and 11 are most common, can cause the occurrence of anogenital warts. Genital infections caused by HPV, are manifested by various clinical entities, from condylomata acuminata, flat warts-condylomata plana, giant condyloma Buschke-Löwenstein, papulosis Bowenoides, as well as some clinical manifestations of intraepithelial lesions (IN) of the genital area. The treatment of genital HPV infection is considered to be a major challenge, the etiological therapy of HPV does still not exist, and the recurrence rate of the disease is common. There are several therapies for the topical treatment of HPV infections, none of which are currently the first line treatment. Since the therapy is sometimes uncomfortable and painful, the recurrence rate is common, treatment
is challenging for both the patient and the physician. Given all, and the availability of HPV vaccine, it is logical that the vaccine is a modern strategy for the prevention of HPV related diseases.
