Abstract

Transplantacija jetre (TJ) uspješna je terapijska metoda za liječenje uznapredovalih kroničnih jetrenih bolesti kao i hepatocelularnog karcinoma. Kao posljedica uspješnog zbrinjavanja akutnih komplikacija i sve dužeg preživljenja pacijenata danas su u fokusu interesa kasne komplikacije nakon TJ. Metabolički sindrom (MS) javlja se u 40-60% pacijenata u prvih 6 mjeseci nakon transplantacije i obuhvaća nekoliko komponenti: pretilost, šećernu bolest, dislipidemiju i hipertenziju. Značajno utječe na preživljenje jer nosi visok rizik za pojavu kardiovaskularnih bolesti, koje su najvažniji uzrok smrtnosti u kasnom poslijetransplantacijskom razdoblju. MS je češći kod pacijenata nakon TJ nego u općoj populaciji, a važnu ulogu u njegovom razvitku imaju imunosupresivni lijekovi. Posljednjih godina na važnosti dobiva i nealkoholna masna bolest jetre (engl. nonalcoholic fatty liver disease – NAFLD), jetrena manifestacija metaboličkog sindroma, koja ne samo što je sve češća indikacija za TJ, već se javlja i kao kasna komplikacija TJ te je povezana s većim rizikom za kardiovaskularne incidente. Ne postoji specifična terapija za MS, stoga je ključna prevencija, rana detekcija i adekvatno liječenje pojedinih komponenti metaboličkog sindroma. Ovaj pregledni rad sveobuhvatno prezentira dosadašnje spoznaje o epidemiologiji, dijagnostici, liječenju, komplikacijama i prognozi metaboličkog sindroma nakon transplantacije jetre.

Abstract (english)

Liver transplantation (LT) is a therapeutic method for end-stage liver diseases and hepatocellular carcinoma. As a result of the successful management of acute complications and prolonged patient survival, transplant medicine focuses on chronic complications after transplant procedure. Metabolic syndrome affects 40-60% of patients after LT within first 6 months after operation and includes several components: obesity, diabetes, dyslipidemia and hypertension. It has a significant influence on patient survival because it increases the risk for cardiovascular diseases, that are the major causes of death in late posttransplant period. Metabolic syndrome occurs more often in patients after transplantation compared to general population, mainly because of the effect of immunosuppressive therapy. In recent years, non-alcoholic fatty liver disease (NAFLD), which is considered to be liver manifestation of metabolic syndrome, has gained the importance. Not only it has become more common as indication for LT, but its prevalence raises late after LT and carries greater risk for cardiovascular incidents. There is no specific therapy for metabolic syndrome, therefore prevention, early detection and adequate treatment of individual components are the key in managing metabolic syndrome. This paper summarizes the current knowledge about epidemiology, diagnosis, treatment, complications and prognosis of metabolic syndrome after liver transplantation.
