Abstract

Smetnje ravnoteže mogu predstavljati veliki problem za pacijenta, a uzrok tome može biti ozbiljne prirode (npr. tumor), te je otkrivanje vestibularnog oštećenja u takvih pacijenata važan korak u daljnjem liječenju. Cilj ovog prospektivnog istraživanja bio je pokazati veće dijagnostičke mogućnosti sinusoidalne harmonične akceleracije (SHA) od obrtajnog pokusa u otkrivanju jednostranog vestibularnog oštećenja. Snimanje je obavljeno metodom videonistagmografije (VNG). Ukupno je prikupljen 41 ispitanik (15 muškaraca i 26 žena), a isključni kriterij je bio izostanak smetnji ravnoteže te nepotpisani informirani pristanak. Ispitanici su podijeljeni u dvije grupe prema nalazu u toplinskom pokusu (simetrična i asimetrična vestibularna podražljivost) te su uspoređivani nalazi obrtajnog pokusa i SHA između grupa (simetrija nistagmičkog odgovora u obrtajnom pokusu s prirastom, pomakom u fazi i simetrijom nistagmičkog odgovora u SHA testu). Asimetrija odgovora u toplinskom pokusu smatrana je patološkom ako je bila veća od 20% što ukazuje na jednostrano vestibularno oštećenje te je utvrđeno 17 takvih ispitanika. Za obrtajni pokus dobivena je osjetljivost od 29% te specifičnost od 88%. U SHA testu značajni prediktivni parametar pokazao se pomak u fazi za frekvenciju 0.32 Hz te osjetljivost i specifičnost za navedeni parametar iznose 77% i 71%. Za prirast pri 0.16 Hz, pomak u fazi pri 0.02 Hz i 0.04 Hz bilježi se trend koji ukazuje na mogući doprinos u određivanju jednostranog vestibularnog oštećenja. Simetrija u SHA nije se pokazala kao prediktivni parametar niti za jednu frekvenciju u određivanju postoji li jednostrano vestibularno oštećenje ili ne. U određivanju modela, odnosno koja kombinacija parametara SHA najviše doprinosi u razlikovanju ispitanika s jednostranim vestibularnim oštećenjem od ostalih, korišteni su logistički regresijski modeli te se kao najbolji model pokazala kombinacija sljedećih parametara: prirast 0.16 Hz, pomak u fazi 0.02 Hz i pomak u fazi 0.32 Hz (AUC-ROC 0.82 naspram AUC-ROC 0.58 za obrtajni stolac naspram AUC-ROC 0.70 za pomak u fazi pri 0.32 Hz). Kombinacijom različitih parametera SHA dobiva se još veći prediktivni kapacitet u razlučivanju pacijenata koji imaju ili nemaju jednostrano vestibularno oštećenje, odnosno postoji određena dijagnostička snaga sadržana u kombinaciji pojedinih parametara, međutim c statistika (ekvivalent AUC-ROC) nam ne govori o tome kako kombinaciju tih parametara ispravno tumačiti u donošenju konačne dijagnoze te je potrebno provesti još istraživanja kako bi se takvo što utvrdilo. Zaključno, SHA ima veću osjetljivost od obrtajnog pokusa u određivanju jednostranog vestibularnog oštećenja, međutim za specifičnost se ne može sa sigurnošću utvrditi je li postoji razlika između ta dva pokusa te je potreban veći broj ispitanika.

Abstract (english)

Disorders of balance can be a major problem for the patient and the cause can be serious (e.g., a tumor) and therefore the detection of vestibular damage in such patients is an important step in further treatment. The aim of this study was to show the better diagnostic possibilities of sinusoidal harmonic acceleration test (SHA) in comparison to step velocity test in detecting unilateral vestibular damage. Recording were obtained by videonystagmography (VNG). The research was prospective. A total of 41 respondents (15 men and 26 women) were collected, and the exclusion criterion was the absence of dizziness and unsigned informed consent. Subjects were divided into two groups according to the findings in the caloric test (symmetric and asymmetric vestibular response) and the results of the step velocity test and SHA were compared between the groups (the symmetry of nystagmic response in the step velocity test with the gain, phase and symmetry of the SHA test). Caloric test values were considered pathological if the asymmetry was greater than 20% indicating unilateral vestibular damage and 17 such subjects were identified. A sensitivity of 29% and a specificity of 88% were obtained for the step velocity test. In the SHA test, a significant predictive parameter proved to be a phase shift for the frequency of 0.32 Hz, and the sensitivity and specificity for this parameter were 77% and 71%. Gain for 0.16 Hz and phase shift for 0.02 and 0.04 Hz showed a trend that indicates a possible contribution to the detetetcion of unilateral vestibular damage. Symmetry in SHA test has not been shown to be a predictive parameter for either frequency in detection whether or not unilateral vestibular damage exists. In determining the model, i.e. which combination of SHA parameters contributes the most in distinguishing subjects with unilateral vestibular damage from others, logistic regression models were used and the combination of the following parameters proved to be the best model: gain at 0.16 Hz, phase at 0.02 Hz and at 0.32 Hz (AUC-ROC 0.82 vs. AUC-ROC 0.58 for step velocity test vs. AUC-ROC 0.70 for phase at 0.32 Hz). The combination of different SHA parameters gives an even greater predictive capacity in distinguishing patients with unilateral vestibular damage from others, i.e. there is a certain diagnostic power contained in the combination of individual parameters, however c statistics (equivalent to AUC-ROC) do not tell how the combination of these parameters should correctly be interpreted in making the final diagnosis and more research is needed to establish such a thing. In conclusion, SHA test has a higher sensitivity than the step velocity test in determining unilateral vestibular damage, however, for specificity it cannot be determined with certainty whether there is a difference between the two test and a larger number of subjects is required.
