Abstract

Popuštanje srca definira se kao sindrom koji nastaje uslijed nemogućnosti srca da
djeluje kao pumpa, a označava abnormalnost strukture i funkcije srca da opskrbi
organizam količinom kisika potrebnom za normalan metabolizam tkiva i organa.
Popuštanje srca prema brzini kliničkih manifestacija dijelimo na akutno i kronično.
Klinička slika kroničnog popuštanja srca je raznolika, te najviše ovisi o stadiju
bolesti i komorbiditetima bolesnika. Postavljanje dijagnoze kroničnog popuštanja
srca zahtijeva ispunjavanje ključnih kriterija koji uključuju simptome i kliničke
znakove popuštanja srca i objektivne dokaze srčane disfunkcije. Ehokardiografski
pregled se danas smatra zlatnim standardom za otkrivanje, procjenu težine i
opsega oštećenja miokarda i ostalih srčanih struktura i postavljanje dijagnoze
kroničnog popuštanja srca. Terapijski pristup liječenju, osim prevencije bolesti
obuhvaća i opće, dijetetske mjere, farmakološku terapiju, mehanička sredstva i
kiruršku terapiju. Biventrikularna elektrostimulacija, odnosno resinkronizacija je
relativno novi terapijski postupak za pacijente sa kroničnim popuštanjem srca.
Resinkronizacija poboljšava cjelokupnu srčanu efikasnost, a kliničke studije su
pokazale statistički značajno smanjenje broja hospitalizacija i smrtnosti, kao i
značajno poboljšanje kvalitete života bolesnika sa dijagnozom kroničnog
popuštanja srca.

Abstract (english)

Heart failure can be defined as a syndrome that occures when heart lose its ability
to function as a pump, term includes an abnormality of cardiac structure and
function to deliver oxygen at a rate commensurate with the requirements of the
metabolizing tissues. Heart failure can be acute and chronic, depending on the
time required for the clinical manifestations of disease. Clinical manifestations of
chronic heart failure are of wide range and they depend on the stage of disease
and patient`s comorbidities. Diagnosis of the chronic heart failure requires meeting
of criteria that include signs and symptoms of disease and objective findings of the
cardiac dysfunction. Echocardiogram is the golden standard in detection,
assessment of disease severity and diagnosis of chronic heart failure. The
therapeutic approach to the treatment, includes prevention, dietary measures,
pharmacological therapy, mechanical devices and surgical therapy. Biventricular
pacing, in another words resynchronization is a relatively new therapeutic
treatment for patients with chronic heart failure. Cardiac resynchronization
improves overall cardiac efficiency, clinical studies have shown a statistically
significant reduction in the number of hospitalizations and mortality rate, as well as
significant improving in the quality of life in patients diagnosed with chronic heart
failure.
