Abstract

Otoskleroza je poremećaj u koštanom metabolizmu koji je ograničen na otičku kapsulu i bazu stapesa. Proces resorpcije i deponiranja koštane mase rezultira fiksacijom slušnih koščica te provodbenim gubitkom sluha. Otoskleroza se pojavljuje u svim dobnim grupama, ali tipična klinička prezentacija bolesti nastupa od 2. do 5. desetljeća života. Tinitus se definira kao prisutnost svjesnog osjeta zvuka u odsutnosti vanjskog zvučnog podražaja te je obično jedan od prvih simptoma otoskleroze, nadalje 65 % bolesnika s otosklerozom pate od klinički značajnog, kroničnog tinitusa prije operacije stapesa. Cilj ovog rada je istražiti utjecaj
stapedotomije na tinitus u bolesnika s otosklerozom. Četrdeset i pet bolesnika s otosklerozom od kojih 35 s prijeoperacijskim tinitusom su uključeni u ovo istraživanje. Svim ispitanicima je učinjena tonalna audiometrija par mjeseci prije i poslije stapedotomije. Također, bolesnicima su postavljena pitanja iz specifičnog upitnika s numeričkom (NRS) skalom za intenzitet tinitusa koji je prilagođen bolesnicima s otosklerozom. Rezultati istraživanja pokazuju da je u 42,85 % bolesnika koji su bili uključeni u istraživanje došlo do potpunog nestanka tinitusa, 25,71 % bolesnika je iskusilo djelomičnu redukciju tinitusa, a 31,43 % bolesnika nije iskusilo nikakvu redukciju tinitusa. Ova studija je pokazala da se stapedotomija može raditi u svrhu djelomičnog ili potpunog rješenja tinitusa u većine bolesnika s otosklerozom.

Abstract (english)

Otosclerosis is alteration in bony metabolism that is exclusive to the otic capsule and stapes footpalate. The process of resorption and redeposition of bone result in fixation of the ossicular chain and conductive hearing loss. Otosclerosis
occurs in all age groups, but the usual clinical presentation varies from second to fifth decade of life. Tinnitus is defined as a conscious awareness of a sound in the absence of an external auditory stimulus and is usually one of the earliest symptoms of otosclerosis; moreover 65 % of all patients with otosclerosis experienced clinically significant, chronic tinnitus before stapes surgery. The objective of this study was to find the effect of stapedotomy on tinnitus for otosclerosis patient. Forty five otosclerosis patient whom thirty five with preoperative tinnitus were enrolled to the study. Pure tone average Air-Bone Gap values was measured few mounts before and few mounts after stapedotomy. Also, the patient was asked questions from the specific question mark with numerical scale (NRS) for intensity of tinnitus adapted to otosclerosis patient. The results of this study showed that 42.85 % of the patient that enrolled in study experienced complete tinnitus reduction, 25.71 % of the patient
experienced sever tinnitus reduction and 31.43 % of the patient was not experienced any tinnitus reduction. In conclusion, this study shows that stapedotomy can be used to achieve full or partial resolution of tinnitus in majority of patients with otosclerosis.
