Abstract

Endometrioza je bolest koja se klinički manifestira dismenorejom, dispareunijom i umanjenom plodnošću. Dijagnoza se postavlja na temelju ginekološkog pregleda,ultrazvučno i laparoskopskom metodom. U obradi neplodnih pacijentica s endometriozom, potrebno je odrediti razinu FSH, LH, E2, te testove procjene ovarijske rezerve: AMH i AFC. Endometrioza se liječi konzervativno i kirurški, a neplodnost koju uzrokuje metodama pomognute oplodnje. Stručnjaci diljem svijeta su podijeljenih mišljenja glede utjecaja endometrioze na ishod IVF postupka, davanja prednosti medikamentoznom liječenju ili kirurškom pristupu te pitanja je li oslabljeni odgovor na ovarijsku stimulaciju rezultat smanjene ovarijske rezerve nastao radi same bolesti ili radi operativnog zahvata liječenja endometrioze.

Abstract (english)

Endometriosis is a disease that manifests clinically as dysmenorrhea, dyspareunia and reduced fertility. Diagnosis is based on a gynecological examination, ultrasound and laparoscopic procedure. In the treatment of infertile women with endometriosis, it is necessary to determine the level of FSH, LH, E2, and evaluation tests in ovarian reserve: AMH and AFC. Endometriosis is treated conservatively and surgically, and infertility is treated by methods of assisted reproduction. Experts around the world are divided opinions about the impact of endometriosis on the outcome of IVF, preference for drug treatment or surgical approach to these questions is whether an attenuated response to ovarian stimulation result of reduced ovarian reserve arising from the disease itself or for the treatment of endometriosis surgery.
