Abstract

Multisistemski upalni sindrom je rijetka i teška komplikacija koje se javlja u vezi s infekcijom SARS-CoV-2. Iako je prvotno opisana kod djece, u literaturi su zabilježeni i slučajevi kod odraslih sa životno ugrožavajućim simptomima. Radi se o stanju pretjerane upale koja zahvaća više organskih sustava, najčešće kardiovaskularni sustav. Ovisno o literaturi, smrtnost iznosi 8-10 % te je za povoljne ishode ključno pravovremeno postavljanje dijagnoze koje omogućuje i pravilno liječenje. Tako se, osim smrtnog ishoda, sprječavaju i potencijalne dugotrajne komplikacije i kronična oboljenja pojedinog organskog sustava.
U radu je prikazan pregled do sada poznatih informacija o MIS-A, kao i kratki pregled osnovne bolesti COVID-19 koji može poslužiti za bolje razumijevanje različitih mehanizama uključenih u nastanak MIS-A.
Također je opisan i slučaj 31-godišnjeg pacijenta koji je inače bio zdrav (prebolio blaži oblik COVID-19 u 2020.), a imao je teške simptome kardiocirkulatornog zatajenja te renalno i hepatalno oštećenje. Iako nije imao anamnestički podatak o nedavnoj infekciji, najvjerojatnije se radi o MIS-A.

Abstract (english)

Multisystem inflammatory syndrome is rare but serious complication reported in association with COVID-19. AT first, it was reported in children, but in the scientific literature there are described cases in adults with life-threatening symptoms. MIS-A is hyperinflammatory state of the organism affecting multiple organ systems at once, most commonly cardiovascular system. Depending on the literature, mortality of MIS-A ranges between 8% and 10% with timely diagnosis and proper treatment being the best predictors of positive outcomes in these patients. In that way, besides fatality, potential complications and chronic disease of respective organ system is prevented as well.
Here, I show overview of available information on MIS-A, until now, as well as brief review of primary disease COVID-19 which can serve to better understand different mechanisms involved in emergence of MIS-A.
Furthermore, it is presented a case of 31 year-old patient with severe cardiocirculatory failure and renal and hepatal injury, who was previously of good health (he had mild COVID-19 in 2020.). Although, there was no anamnestic evidence of recent SARS-CoV-2 infection, it is probably a case of MIS-A.
