Abstract

Cilj: cilj ovoga rada bio je utvrditi učestalost i stopu postavljanja dijagnoze prirođenih srčanih bolesti u živorođene novorođenčadi uz pomoć UZV u tercijarnom centru zdravstvene zaštite, KB ''Sveti Duh'' u Zagrebu, kao i učestalost različitih oblika prirođenih srčanih bolesti u razdoblju od 1. siječnja 2016. do 31. prosinca 2020. godine.
----- Metode i rezultati: provedeno je retrospektivno istraživanje, a podaci su skupljeni uvidom u povijesti bolesti majki i novorođenčadi u KB ''Sveti Duh''. Od ukupno 13 932 poroda kod 42 novorođenčeta dijagnosticirana je prirođena srčana bolest. Ukoliko isključimo dijagnozu perzistentnog Botallijeva duktusa (n=4), te srčane bolesti nastale kao posljedica stanja majke (n=2), od 36 novorođenčadi u 31 novorođenčeta bolest je dijagnosticirana prenatalno (86%) a u 5 novorođenčadi postnatalno (14%). Prevalencija je iznosila 30 na 10 000 poroda (3,0‰).
----- Zaključak: Stopa postavljanja dijagnoze je značajno veća u usporedbi sa stopom detekcije u drugim zemljama Europe (1) (2). Prevalencija prirođenih srčanih grešaka značajno je manja od prevalencije na razini Europe objavljene u EUROCAT studiji za razdoblje od 2016. do 2019. godine (3). Potrebno je provesti dodatne mjere edukacije i algoritme pregleda kako bi uspješnost prenatalnog otkrivanja prirođenih srčanih bolesti u KB ''Sv. Duh'' dostigla one u najboljim centrima Europe i svijeta.

Abstract (english)

Aims: To assess frequency and success rate of prenatal diagnosis of congenital heart diseases (CHDs) in live-born neonates using ultrasound as well as occurrence of different types of congenital heart diseases in the center of tertiary medical care, the Clinic for gynecology and obstetrics at Clinical Hospital ''Sveti Duh'' Zagreb, Croatia, in the period from January 1st 2016 till December 31st 2020.
----- Methods: A retrospective study was conducted. Data were collected by reviewing the medical history of both mothers and newborns.
----- Results: 13932 childbirths were performed, and 42 newborns were diagnosed with congenital heart disease. Excluding the diagnoses of persistent ductus Botalli (n=4) and CHDs arising as a consequence of maternal condition (n=2), out of 36 newborns, 31 were diagnosed prenatally (86%) and 5 postnatally (14%). The prevalence of CHDs was 30 per 10000 births (3,0 ‰).
----- Conclusion: The rate of prenatal diagnosis of CHDs is significantly higher compared with other European countries (1,2). The prevalence of CHDs was substantially lower than that of the European average published in the EUROCAT study for the period from 2016 to 2019 (3). Additional education and examination algorithms need to be implemented to achieve more successful rates of prenatally diagnosed CHDs to match those in the best centers across Europe and the world.
