Abstract

KOPB i astma jedne su od najučestalijih bolesti, a bilježi se daljnji porast njihove prevalencije. I KOPB i astma, kronične opstruktivne bolesti pluća, heterogene su bolesti koje ponajprije zahvaćaju respiratorni sustav, ali ujedno narušavaju i kvalitetu života i sveukupno zdravlje pacijenata. Ranije se smatralo da se težina KOPB-a i astme zasniva isključivo na težini bronhoopstrukcije procijenjene testovima plućne funkcije, no pokazalo se kako stupanj bronhoopstrukcije ne odražava u potpunosti jačinu simptoma i kvalitetu života. Iz navedenog razloga proizašli su upitnici za praćenje kontrole KOPB-a i astme koji sagledavaju više aspekata pacijentove bolesti. Upitnici u praćenju kroničnih opstruktivnih bolesti pluća koji se danas koriste u kliničkoj praksi služe za stupnjevanje težine bolesti, praćenje kontrole bolesti i pomoć u odabiru modaliteta liječenja.
Za praćenje kontrole KOPB-a razvijeno je nekoliko upitnika među kojima su najšire upotrebljavani mMRC skala dispneje, upitnik za procjenu kronične opstruktivne bolesti pluća (CAT) te klinički upitnik za KOPB (CCQ). U praćenju kontrole astme koriste se upitnik o kontroli astme (ACQ) i test kontrole astme (ACT) te verzija ACT-a prilagođena praćenju kontrole astme u djece (c-ACT). Brojna su istraživanja proučavala psihometrijska svojstva navedenih upitnika te su za većinu svojstava upitnici pokazali dobre karakteristike. Iz tih razloga Globalna inicijativa za kroničnu opstruktivnu bolest pluća (GOLD) i Globalna inicijativa za astmu (GINA) preporučuju njihovo korištenje u svakodnevnoj kliničkoj praksi.

Abstract (english)

COPD and asthma are one of the most common diseases, and there is a further increase in their prevalence. Both COPD and asthma, chronic obstructive pulmonary diseases, are heterogeneous diseases that primarily affect the respiratory system, but also impair the quality of life and overall health of patients. Previously, the severity of COPD and asthma was thought to be based solely on the severity of bronchoobstruction assessed by lung function tests, but the degree of bronchoobstruction has been shown not to fully reflect symptom severity and quality of life. For this reason, questionnaires have been developed to monitor the control of COPD and asthma, which look at several aspects of the patient's disease. Questionnaires in the monitoring of chronic obstructive pulmonary disease used today in clinical practice serve to grade the severity of the disease, monitor disease control and assist in the selection of treatment modalities.
Several questionnaires have been developed to monitor COPD control, including the most widely used mMRC dyspnea scale, the COPD Assessment Test (CAT) and the COPD Control Questionnaire (CCQ). The Asthma Control Questionnaire (ACQ) and the Asthma Control Test (ACT) and the ACT version adapted for monitoring asthma control in children (c-ACT) are used to monitor asthma. Numerous studies have examined the psychometric properties of these questionnaires and for most properties the questionnaires have shown good characteristics. For these reasons, the Global Initiative for Chronic Obstructive Pulmonary Disease (GOLD) and the Global Initiative for Asthma (GINA) recommend their use in everyday clinical practice.
