Utjecaj biofilma kod stafilokokne infekcije oko čeličnih i titanijskih implantata

Abstract

Najveći značaj u stvaranju stafilokoknog biofilma pridaje se ica genu. Utjecaj stvaranja biofilma na osjetljivost različitih implantata prema stafilokoknoj infekciji nije potpuno jasan. Interakcija biofilm pozitivnih i biofilm negativnih S.aureus i S.epidermidis s površinama titanija, glatkog te hrapavog čelika proučavana je in vitro i in vivo. Korišten je „tissue cage“ model na mišu za perioperacijsku i poslijeoperacijsku inokulaciju pored inokulacije u in vitro uvjetima i proučavanja morfologije biofilma na površinama implantata pomoću pretražnog elektronskog mikroskopa.Biofilm pozitivni S.epidermidis adherirao je jednako ili snažnije za površine svih implantata od biofilm pozitivnog S.aureus. Biofilm pozitivni stafilokoki pokazali su višeslojni biofilm. 300 cfu (jedinica koje tvore kolonije) biofilm pozitivnih i negativnih S.aureus izazvalo je infekciju s visokim koncentracijama planktonskih bakterija nakon 8 dana in vivo, pri čemu je je samo 0,89% ukupnih bakterija bilo adherentno. Za izazivanje poslijeoperacijske infekcije sa S.epidermidis bilo je potreban inokulum od 106 cfu, kod svih vrsta implantata koncentracija planktonskih bakterija pala je ispod 100 cfu, a adherentne bakterije bile su niskih koncentracija kod biofilm pozitivnih S.epidermidis i odsutne kod biofilm negativnih. Perioperacijska inokulacija rezultirala je višim koncentracijama planktosnkih bakterija od poslijeoperacijske inokulacije, kod titanijskih implantata adherentne bakterije bile su u većoj koncentraciji nakon inokulacije biofilm pozitivnim sojem od biofilm negativnog soja. Vrsta metala slabo je utjecala na osjetljivost prema stafilokoknoj infekciji. Postojanje ica gena imalo je snažan utjecaj in vitro i slab utjecaj in vivo. S.epidermidis pokazao se patogenijim kada je inokuliran za vrijeme implantacije nego kada je inokuliran nakon implantacije.

Abstract (english)

Today ica is considered the major operon responsible for staphylococcal biofilm. The effect of biofilm on susceptibility to staphylococcal infection of different implant materials in vivo is unclear. The interaction of ica-positive (wild type, [wt]) and ica-negative Staphylococcus (S.) aureus and S. epidermidis strains with titanium, smooth and rough stainless steel surfaces was studied by scanning electron microscopy in vitro and in a subcutaneous mouse tissue cage model during 2 weeks following peri- or post-operative inoculation in vivo. In vitro S. epidermidis wt adhered equally and more strongly than S. aureus wt to all materials. Wt but not ica- strains of both species showed multilayered biofilm. In vivo 300 cfu wt and ica- S. aureus led in all metal cages to an infection with high numbers of planktonic bacteria after 8 days; only 0.89 % of total wt and ica- S. aureus were adherent. In contrast, for postoperative infection with S. epidermidis 106 cfu of the wt and ica- strain were required; in all metal types planktonic bacterial numbers dropped to <100 cfu, adherent cfu were low in wt- and absent in ica--infected cages after 14 days. Perioperative inoculation resulted in slower clearance than postoperative inoculation and in titanium cages adherent wt bacteria survived in higher numbers than ica- bacteria. In conclusion, the metal played a minor role regarding susceptibility to and persistence of staphylococcal infection; the presence of ica genes had a strong effect on biofilm in vitro and a weak effect in vivo; and S. epidermidis was more pathogenic when introduced during than after implantation.
