Abstract

Trudnoća je fiziološko stanje udruženo s brojnim endokrinološkim, imunološkim, metaboličkim i vaskularnim promjenama koje dovode do fizioloških i patoloških promjena na koži. Fiziološke kožne promjene javljaju se u više od 90% trudnoća, za razliku od dermatoza specifičnih za trudnoću koje se javljaju u 14% trudnoća. Dermatoze specifične za trudnoću heterogena su skupina upalnih dermatoloških bolesti čiji je zajednički simptom svrbež. Atopijska erupcija u trudnoći najčešća je dermatoza specifična za trudnoću, a slijedi ju polimorfna erupcija u trudnoći s incidencijom od 1:300. To su bezopasna stanja koja ne nose rizik ni za majku ni za dijete. Intrahepatična kolestaza u trudnoći, pemphigoid gestationis i pustularna psorijaza u trudnoći javljaju se rjeđe, ali nose rizik za fetus te zahtijevaju odgovarajuće antenatalno praćenje. Ključno je pravovremeno prepoznavanje i međusobno razlikovanje ovih kožnih bolesti uz pomoć kliničke slike i dijagnostičkih pretraga kako bi se pružila adekvatna antenatalna skrb i liječenje od strane multidisciplinarnog tima koji uz specijaliste dermatologije uključuje i specijaliste ginekologije i opstetricije. Trenutno još uvijek etiologija i patogeneza ove skupine bolesti nisu u potpunosti jasne, te zbog toga postoji potreba za daljnjim istraživanjem kako bi se bolje razumjeli uzroci i mehanizmi ovih stanja i razvile učinkovitije metode liječenja.

Abstract (english)

Pregnancy is a physiological state associated with numerous endocrinological, immunological, metabolic and vascular changes that lead to physiological and pathological changes in the skin. Physiological skin changes occur in more than 90% of pregnancies, in contrast to pregnancy-specific dermatoses that affect 14% of pregnancies. Dermatoses specific to pregnancy are a heterogeneous group of inflammatory dermatological diseases characterized by itching as a common symptom. The most common dermatosis specific to pregnancy is atopic eruption in pregnancy, followed by polymorphic eruption in pregnancy with an incidence of 1:300. These conditions are harmless and do not pose any risks to the mother or the child. However, less common conditions like intrahepatic cholestasis in pregnancy, pemphigoid gestationis and pustular psoriasis in pregnancy can pose risks to the fetus and require appropriate antenatal monitoring. Timely recognition and differentiation of these skin diseases with the help of clinical picture and diagnostic tests are crucial for providing adequate antenatal care and treatment by a multidisciplinary team that, in addition to dermatology specialists, also includes gynecology and obstetrics specialists. Currently, the etiology and pathogenesis of this group of diseases is not yet completely clear, therefore, further research is needed in order to better understand their causes and mechanisms, and develop more effective treatment methods.
