Abstract

Šećerna bolest je najčešća metabolička bolest u općoj populaciji obilježena poremećajem metabolizma ugljikohidrata. Poremećaj lučenja ili djelovanja inzulina dovodi do povišene razine glukoze u krvi (hiperglikemije) kao osnovnog obilježja bolesti. Šećerna bolest u trudnoći povezana je s povećanim rizikom brojnih komplikacija kako u majke tako i u djeteta, poglavito u slučajevima kada hiperglikemija nije odgovarajuće tretirana. Velika većina mogućih komplikacija od spontanih pobačaja do teških fetalnih anomalija razvoja, porodnih ozljeda zbog makrosomije, te asfiksije, respiratornih i metaboličkih komplikacija u novorođenčadi može biti prevenirana pravodobnim terapijskim intervencijama. U oboljelih, prekoncepcijska regulacija bolesti i dobra kontrola tijekom trudnoće temeljni su preduvjeti smanjenja rizika navedenih komplikacija. Od posebnog značaja je detektiranje onih koje će tek u trudnoći razviti poremećaj metabolizma glukoze, često subjektivno asimptomatskih, ali ne i bez rizika za potomstvo. Sustavni probir na hiperglikemiju u trudnoći i njezina regulacija smanjuju rizike nepoželjnih komplikacija u novorođenčadi.

Abstract (english)

Diabetes is the most common metabolic disease in the general population characterized by a disorder of carbohydrate metabolism. Impairment of insulin secretion or action leads to elevated blood glucose levels (hyperglycemia) as a basic feature of the disease. Diabetes in pregnancy is associated with an increased risk of numerous complications in both mother and child, especially in cases where hyperglycemia is not properly treated. The vast majority of possible complications from spontaneous abortions to severe fetal anomalies of development, birth injuries due to macrosomia, and asphyxia, respiratory and metabolic complications in newborns can be prevented by timely therapeutic interventions. In patients, preconception regulation of the disease and good control during pregnancy are fundamental prerequisites for reducing the risk of these complications. Of particular importance is detection of those who will develop a disorder of glucose metabolism during pregnancy, often subjectively asymptomatic, but not without risks for the offspring. Systematic screening for hyperglycemia in pregnancy and its regulation reduce the risks of undesirable complications in newborns.
