Abstract Hitna psihijatrija obuhvaća kliničku primjenu psihijatrije u hitnim stanjima. U takvim situacijama, određeni psihofarmaci mogu izazvati psihijatrijske sindrome koji zahtijevaju hitnu evaluaciju i liječenje. Također, postoji rizik od ozbiljnih neuroloških nuspojava povezanih s upotrebom psihofarmaka, kao što su ekstrapiramidalni simptomi, neuroleptički maligni sindrom i serotoninski sindrom.
Hitna psihijatrija je specijalizirano područje kod koje je ključno brzo reagiranje, procjena sigurnosti pacijenta i pružanje odgovarajuće terapije. Ekstrapiramidalni simptomi su neurološki poremećaji koji se mogu javiti kao nuspojava uzimanja određenih psihofarmaka, poput antipsihotika. Oni se manifestiraju poremećajima pokreta i dovode do onesposobljenosti i ometanju svakodnevnih aktivnosti. Neuroleptički maligni sindrom je rijedak, ali potencijalno životno ugrožavajući sindrom povezan s upotrebom psihofarmaka. Očituje se mišićnim rigiditetom, agitacijom, te neurološkim simptomima i simptomima disfunkcije autonomnog živčanog sustava. Serotoninski sindrom je ozbiljan neurološki poremećaj koji se može javiti kao reakcija na prekomjernu stimulaciju serotoninskog sustava. Ovo stanje obuhvaća simptome poput hipertermije, promjene mentalnog statusa, te autonomne i neuromuskularne hiperaktivnosti. Svi navedeni sindromi zahtijevaju prekid uzimanja uzročnih lijekova, hitnu medicinsku procjenu i ovisno o težini individualizirani terapijski pristup.
Rani prekid uporabe, hitna medicinska intervencija i simptomatsko liječenje su ključni zapovoljan ishod i smanjenje komplikacija. Pravovremena dijagnoza, agresivno liječenje ipoboljšanje terapijskih postupaka kroz medicinski napredak će unaprijediti brigu o pacijentima. Edukacija i svijest zdravstvenih stručnjaka su također važni, doprinoseći boljem razumijevanju i upravljanju ovim hitnim psihijatrijskim stanjima.
Abstract (english) Emergency psychiatry encompasses the clinical application of psychiatry in emergency situations. In such cases, certain psychotropic medications can induce psychiatric syndromes that require urgent evaluation and treatment. Additionally, there is a risk of serious neurological side effects associated with the use of psychotropic drugs, such as extrapyramidal symptoms, neuroleptic malignant syndrome and serotonin syndrome.
Emergency psychiatry is a specialized field that requires prompt response, assessment of patient safety and the provision of appropriate therapy. Extrapyramidal symptoms are neurological disorders that can occur as a side effect of certain psychotropic medications, such as antipsychotics. They manifest as movement disorders and lead to disability and impairment of daily activities. Neuroleptic malignant syndrome is a rare but potentially life threatening syndrome associated with the use of psychotropic drugs. It presents with muscle rigidity, agitation, neurological symptoms and dysfunction of the autonomic nervous system. Serotonin syndrome is a serious neurological disorder that can occur in response to excessive stimulation of the serotonin system. This condition includes symptoms such as hyperthermia, changes in mental status, autonomic and neuromuscular hyperactivity. All the mentioned syndromes require discontinuation of the causative medications, urgent medical assessment and depending on the severity, an individualized therapeutic approach.
Early discontinuation of medication, prompt medical intervention and symptomatic treatment are crucial for favorable outcomes and reducing complications. Timely diagnosis, aggressive therapy and improvement in therapeutic approaches through medical advancements will enhance patient care. Education and awareness among healthcare professionals are also important, contributing to better understanding and management of these psychiatric emergencies.
