Učestalost pridruženih autoimunih bolesti u bolesnika sa šećernom bolešću tipa 1

Abstract

Šećerna bolest tipa 1 (ŠB1) autoimuna je bolest obilježena hiperglikemijom koja se javlja zbog nedostatne sekrecije inzulina uslijed imunološki posredovane destrukcije β-stanica gušterače. S obzirom na autoimunu podlogu bolesti, češće je udruženo pojavljivanje s drugim autoimunim bolestima zbog djelomično zajedničkih genetskih i okolišnih čimbenika.

Cilj je ovoga rada analiza učestalosti i vrsta pridruženih autoimunih bolesti u bolesnika sa ŠB1 liječenih u KBC-u Zagreb i usporedba s rezultatima dosadašnjih istraživanja.

Podaci su prikupljeni putem bolničkog informacijskog sustava od 404 pacijenta sa ŠB1 liječenih na Zavodu za endokrinologiju KBC-a Zagreb u razdoblju od 2011. do 2023. godine. Analizirani podaci obuhvaćaju opće karakteristike bolesnika, podatke o regulaciji glikemije, zatim o dijabetičkim komplikacijama te postojanju pridruženih autoimunih bolesti.

Analizom podataka utvrđeno je da je od 404 bolesnika bilo 194 (48%) osoba muškog spola i 210 (52%) osoba ženskog spola te je prosječna dob iznosila 35±14 godina. Dijabetičke komplikacije pronađene su u 94 (23,3%) bolesnika te je najčešća bila dijabetička retinopatija. Ukupno je pronađeno 118 pridruženih autoimunih bolesti u 100 bolesnika te je njihova prevalencija bila 24,8%, a najčešće među njima bile su autoimuni tireoiditis i celijakija.

Zaključno, ovo istraživanje potvrđuje veću učestalost pridruženih autoimunih bolesti u bolesnika sa ŠB1 u odnosu na opću populaciju, te se kao najčešće pridružene bolesti izdvajaju autoimuni tireoiditis i celijakija, sukladno rezultatima ostalih sličnih istraživanja.

Abstract (english)

Type 1 diabetes mellitus (T1D) is an autoimmune disease characterized by hyperglycemia due to insufficient insulin secretion caused by immune-mediated destruction of pancreatic β-cells. Due to the autoimmune nature of the disease, the occurrence of other autoimmune diseases is more common, partly due to shared genetic and environmental factors.

The aim of this study is to analyze the incidence and types of associated autoimmune diseases in patients with T1D treated at the University Hospital Center Zagreb and compare the results with previous research.

Data were collected through the hospital information system from 404 patients with T1D treated at the Department of Endocrinology, UHC Zagreb, between 2011 and 2023. The analyzed data include general patient characteristics, information on glycemic control, diabetic complications, and associated autoimmune diseases.

Data analysis revealed that out of 404 patients, 194 (48%) were male, and 210 (52%) were female, with an average age of 35±14 years. Diabetic complications were found in 94 (23.3%) patients, with diabetic retinopathy being the most common. A total of 118 associated autoimmune diseases were found in 100 patients, with a prevalence of 24.8%, with autoimmune thyroiditis and celiac disease being the most frequent among them.

In conclusion, this study confirms a higher incidence of associated autoimmune diseases in patients with T1D compared to the general population, with autoimmune thyroiditis and celiac disease being the most common, consistent with the results of other similar studies.
