Abstract

Kardiovaskularne bolesti globalno zauzimaju prvo mjesto na listi uzroka smrti te izrazito opterećuju zdravstveni sustav. Periferna arterijska bolest, kao jedan od oblika kardiovaskularnih bolesti, obilježena je stenozom odnosno okluzijom arterija donjih ekstremiteta s posljedičnim ugrožavanjem perfuzije i vitalnosti tkiva. Jedan od najvažnijih rizičnih čimbenika za razvoj periferne arterijske bolesti je ateroskleroza koja, zajedno sa ostalim čimbenicima rizika poput hipertenzije, šećerne... More bolesti, dislipidemije i nezdravih životnih navika, ubrzava razvoj odnosno progresiju periferne arterijske bolesti. Klaudikacije, grčenje mišića, sporo zarastanje rana jedni su od simptoma koji upućuju na perifernu arterijsku bolest. S ciljem olakšanja simptoma i izbjegavanja amputacije dijela donjeg ekstremiteta, danas jedna od najčešćih operacija koja se izvodi povodom uklanjanja okluzije odnosno održavanja adekvatne perfuzije kroz arteriju je endarterektomija. Jedno od predilekcijskih mjesta razvoja aterosklerotskih lezija na donjim ekstremitetima je zajednička femoralna ili bedrena arterija. Nakon izvođenja svih arteriotomija, time i femoralne arteriotomije, dokazano je kako zatvaranje arterije zakrpom daje bolje rezultate u vidu smanjenja rekurentne restenoze u odnosu na direktno zatvaranje rubova arterije. Postoje brojni materijali od kojih su zakrpe izrađene, međutim određeni materijali pokazuju bolje rezultate u vidu razvoja smanjenog broja postoperativnih komplikacija. Svinjski perikard jedan je od bioloških materijala koji se danas primjenjuje u vaskularnoj kirurgiji za izradu graftova odnosno zakrpi. U ovom diplomskom radu razmatraju se način obrade, primjena i prednosti zakrpi od svinjskoga perikarda u usporedbi sa zakrpama izrađenih od drugih materijala. Less

Abstract (english)

Cardiovascular diseases globally are the most common cause of death and greatly burden the healthcare system. Peripheral arterial disease, as one type of cardiovascular diseases, is characterized by stenosis or occlusion of the arteries in lower extremities with the consequent endangerment of tissue perfusion and vitality. One of the most important risk factors for the development of peripheral arterial disease is atherosclerosis, which, together with other risk factors such as... More hypertension, diabetes mellitus, dyslipidemia and unhealthy life habits, accelerates the development and progression of peripheral arterial disease. Claudications, muscle spasms, slow wound healing are some of the symptoms that indicate peripheral arterial disease. With the aim of alleviating symptoms and avoiding amputation of a part of the lower limb, today one of the most common operations performed to remove the occlusion and maintain adequate perfusion through the artery is endarterectomy. One of the predilection sites for the development of atherosclerotic lesions in lower extremities is the common femoral artery. After performing all arteriotomies, including the femoral arteriotomy, it was proven that the closure of the artery with a patch gives better results in the form of reduction of recurrent restenosis compared to the direct artery closure. There are numerous materials patches can be made from, however some materials have shown reduced rates of postoperative complications in their use. Porcine pericardium is one of biological materials used today in vascular surgery for making grafts or patches.
