Abstract

Kronične upalne bolesti crijeva (IBD) - Crohnova bolest, ulcerozni kolitis i
neklasificirana upalna bolest crijeva sve se češće dijagnosticiraju u dječjoj i adolescentskoj
dobi. Ove bolesti se razlikuju po lokalizaciji upale. Kod ulceroznog kolitisa bolest je ograničena
na debelo crijevo, dok kod Crohnove bolesti upalom može biti zahvaćen cijeli gastrointestinalni
trakt. Crijevo je organ koji se nalazi pod neprestanom imunološkom stimulacijom mikroba i
antigena iz hrane. Kao odgovor na to dolazi do tzv. "fiziološke“ upale sluznice. Kod razvoja
upalnih bolesti patološka upala zamjenjuje fiziološku. I dalje je nejasno je li riječ o
abnormalnom odgovoru na uobičajene enteričke antigene ili o normalnom odgovoru na još
neidentificirane mikrobe. Simptomi koje ima pacijent su važan indikator upale i aktivnosti same
bolesti, ali su istovremeno i subjektivni, mogu biti potencirani drugim neupalnim činiteljima
kao što su intestinalne strikture, malapsorpcija žučnih soli, nepodnošenje laktoze i brojnim
drugima. Rano postavljanje dijagnoze je važno zbog boljeg terapijskog učinka. Klasičnu
terapiju upalnih bolesti crijeva čine aminosalicilati, kortikosteroidi te imunomodulatorni
lijekovi, a u novije vrijeme i biološka terapija. Konvencionalna terapija i dalje je prva i osnovna
linija kod terapije upalnih bolesti crijeva svih lokalizacija i aktivnosti. Glavni ciljevi terapije
su ublažavanje simptoma, postizanje remisije i kvalitetan život bolesnika.

Abstract (english)

Chronic inflammatory bowel disease (IBD) - Crohn's disease, ulcerative colitis and unclassified
inflammatory bowel disease are increasingly being diagnosed in children and adolescents.
These diseases differ in the localization of inflammation. In ulcerative colitis it is limited to the
large intestine, while in Crohn's disease inflammation can affect the entire gastrointestinal tract.
The gut is an organ that is under constant immune stimulation of microbes and antigens from
food. In response to this comes the so-called. "physiological" mucositis. In the development of
inflammatory diseases, pathological inflammation replaces physiological inflammation. It is
further unclear whether this is an abnormal response to a common enteric antigen or a normal
response to as yet unidentified microbes. Symptoms of the patient as an important indicator of
inflammation and activity of the same disease, but also subjective, can be exacerbated by other
non-inflammatory factors such as intestinal strictures, malabsorption of bile salts, lactose
intolerance, constipation and many others. Early diagnosis is important for better therapeutic
effect. Classical therapy of inflammatory bowel disease consists of aminosalicylates,
corticosteroids and immunomodulatory drugs and recently biologic therapy. Conventional
therapy is still the first and basic line in the treatment of inflammatory bowel disease of all
localizations and activities.
