Abstract

Plućne i srčane bolesti se manifestiraju intolerancijom napora koja se definira kao nesposobnost uspješnog izvršenja fizičkog zadatka kojeg zdrava osoba može izvršiti bez poteškoća. Parametri plućne i srčane funkcije mjereni u mirovanju nisu primjereni pokazatelji stupnja intolerancije napora. Spiroergometrija (Cardiopulmonary exercise testing- CPET) je nova metoda koja pruža jedinstvenu mogućnost istovremenog proučavanja staničnog, kardiovaskularnog i odgovora respiratornog sustava pri točno određenim uvjetima metaboličkog stresa. No, primjena spiroergometrije u pedijatrijskoj populaciji se razlikuje u mnogim aspektima od primjene ove dijagnostičke metode u adultnoj kardiologiji. Kardiopulmonalni odgovor u djece tijekom testa opterećenja se prezentira parametrima različitim od onih u odrasloj dobi, a različit je i spektar indikacija. Spiroergometrija i ostale metode koje procjenjuju funkcionalnu sposobnost organizma u kretanju imaju središnju ulogu u ocjeni aerobnog kapaciteta u djece sa prirođenim srčanim grješkama na temelju čega se formiraju smjernice o daljnjem liječenju, preporuke o fizičkoj aktivnosti u sportu, rehabilitaciji kao i pri profesionalnom usmjeravanju.

Abstract (english)

Lung and heart disease are manifested as intolerance of physical effort defined as inability to successfully execution of the physical task, which healthy person can manage without any difficulties. Measuring parameters of lung and heart function at rest are not appropriate indices of intolerance degree. Cardiopulmonary exercise testing (CPET) is a novel method that offers the unique opportunity to study the cellular, cardiovascular and respiratory systems responses simultaneously under precise conditions of metabolic stress. CPET in paediatric cardiology differs in many aspects from the tests performed in adult cardiology. Children's cardiovascular responses during exercise testing present different characteristics and range of indications is different. Spiroergometry and other metods which measure functional ability of human body in movement have the main role in assessment of aerobic capacity in children born with congenital heart defect, based on this results are formed guidelines about future medical treatment, recommendations on physical activities in sports, about possible rehabilitation and professional orientation in jobs.
