Abstract

Opekline su ozljede uzrokovane djelovanjem topline, obilježene lokalnim, ali i sistemskim simptomima. Opeklinski šok nastaje kao posljedica teških opeklina i obuhvaća kompleksne patofiziološke promjene. Patofiziologija opeklinskog šoka uključuje hemodinamske promjene, poremećaj mikrocirkulacije i metabolički i imunološki odgovor organizma. Upalni medijatori i citokini, djelujući na kapilare, uzrokuju vazodilataciju, povećanu permeabilnost, gubitak proteina plazme i posljedično hipovolemiju. Zbrinjavanje opeklinskog šoka uključuje klasifikaciju opeklina određivanjem površine opečene kože i težine opekline i volumnu resuscitaciju velikim količinama kristaloidnih otopina prema unaprijed definiranim protokolima za zbrinjavanje opeklinskog šoka. Parkland i modificirana Brookeova formula najčešći su korišteni protokoli. Iako je zbrinjavanje opeklinskog šoka značajno napredovalo, suvremene metode i dalje imaju određena ograničenja, zbog čega se kontinuirano istražuju novi pristupi liječenju, poput svježe smrznute plazme, vitamina C i kontinuirane nadomjesne bubrežne terapije kako bi se smanjila stopa komplikacija i skratilo vrijeme hospitalizacije.

Abstract (english)

Burns are injuries caused by the action of heat, characterized by both local and systemic symptoms. Burn shock occurs as a result of severe burns and encompasses complex pathophysiological changes. The pathophysiology of burn shock includes hemodynamic changes, disruption of microcirculation, and the body's metabolic and immune response. Inflammatory mediators and cytokines, acting on the capillaries, cause vasodilation, increased permeability, protein loss from plasma, and consequent hypovolemia. The management of burn shock involves the classification of burns by determining the surface area of burned skin and the severity of the burn, as well as volume resuscitation with large amounts of crystalloid solutions according to predefined protocols for burn shock management. The Parkland and modified Brooke formulas are the most commonly used protocols. Although the management of burn shock has significantly advanced, current methods still have certain limitations, which is why new treatment approaches, such as fresh frozen plasma, vitamin C, and continuous renal replacement therapy, are continuously being researched to reduce the complication rate and shorten hospitalization time.
