Abstract

Politrauma se definira kao ozljeda pri kojoj su zahvaćena najmanje dva organska sustava ili dva različita organa pri čemu jedna od ozljeda ugrožava život bolesnika. U današnje vrijeme najčešći uzrok politraume su prometne nesreće. Učestao je uzrok smrti ponajviše kod osoba mlađe i srednje životne dobi. Brza i točna dijagnostička obrada, u skladu sa smjernicama, ima bitnu ulogu u zbrinjavanju politraumatiziranih bolesnika. U dijagnostičkoj obradi, uz klinički pregled, koriste se i razne radiološke metode. Danas se najčešće koristi snimanje kompjutoriziranom tomografijom glave, vrata, toraksa, abdomena i zdjelice, ali i ultrazvučni pregled, rendgensko snimanje te snimanje magnetnom rezonancijom. Kod ozljede glave dijagnostička obrada, u skladu s prihvaćenim smjernicama, započinje snimanjem kompjutoriziranom tomografijom. U evaluaciji ozljeda kralježnice korisno je rendgensko snimanje, međutim upotreba kompjutorizirane tomografije i magnetske rezonancije pruža mnogo više dodatnih korisnih informacija. U politraumi često dolazi do ozljeđivanja prsnog koša, a za dijagnosticiranje tih ozljeda korisno je i rendgensko snimanje, u određenim slučajevima i ultrazvučna obrada, iako se danas najčešće pristupa snimanju kompjutoriziranom tomografijom. Kod abdomena, kao i kod prsnog koša, bitno je misliti na učestalu koegzistenciju ozljeda. U evaluaciji ozljeda abdomena koristimo i ultrazvučnu obradu i obradu kompjutoriziranom tomografijom. Za dijagnozu ozljeda ekstremiteta koristimo rendgensko snimanje te kompjutoriziranu tomografiju.

Abstract (english)

Polytrauma or multiple trauma is defined as an injury involving at least two organ systems or two different organs, with one of the injuries endangering the patient's life. Today, the most common cause of polytrauma is a traffic accident. It is a frequent cause of death mostly in younger and middle-aged persons. Fast and accurate diagnostic evaluation, in accordance with the guidelines, plays an important role in the treatment of a politraumatized patients. After clinical examination, various radiological methods are used in diagnostic evaluation. Most commonly used is whole-body computed tomography, but also ultrasound, x-ray and magnetic resonance imaging. In head injury, diagnostic process, in accordance with accepted guidelines, begins with computed tomography. In the evaluation of spinal injuries, x-rays are helpful, but the use of computed tomography and magnetic resonance imparts information that is more useful. In polytrauma, chest injuries are common, and to diagnose these injuries x-rays are also helpful, in some cases ultrasonic processing can be useful, even though today it is most commonly assessed by computed tomography. In the abdomen, as well as in the chest, it is important to think of the frequent coexistence of injuries. In the evaluation of abdominal injury, we also use ultrasonic and computed tomography imaging. To diagnose the injury of the extremities, we use X-rays and computed tomography.
