Abstract

Vestibularni neuronitis akutna je upala vestibularnog živca, a karakteriziran je akutnim napadajem teške vrtoglavice, mučnine i nestabilnosti te odsutnošću neuroloških i slušnih deficita. Najčešće je virusne etiologije, a mogući su i autoimuni i vaskularni uzroci. Anamneza i fizikalni pregled obično su dovoljni za dijagnozu, s naglaskom na isključenje potencijalnih životno ugrožavajućih centralnih uzroka. Neuroradiološke metode većinom nisu potrebne, iako bi se CT trebao uzeti u obzir za kronične bolesti uha te MR za slučajeve koji ukazuju na centralni uzrok (CVI moždanog debla ili malog mozga). Terapija akutne faze vestibularnog neuronitisa primarno je farmakološka, dok je dugoročno liječenje osmišljeni da poboljša vestibularnu kompenzaciju. Prognoza je dobra i kod većine dolazi do potpunog oporavka vestibularnog osjetila bez ponovnih napadaja bolesti.

Abstract (english)

Vestibular neuritis is an acute inflammatory disorder of the vestibular nerve, characterized by acute onset vertigo, nausea and imbalance, and with no neurological or auditory deficits. It is most commonly of viral origin with other possible etiologies being autoimmune and vascular. History and physical exam are usually enough to make a diagnosis, with a focus on excluding potential life-threatening central conditions. Neuroradiological methods are mostly unnecessary, however a CT scan should be considered for chronic ear disease and MRI for cases where there is suspicion of a central lesion (brainstem or cerebellum CVI). Primary therapy for acute vestibular neuritis is pharmacological, while long term vestibular compensation is promoted by rehabilitation. Outcomes are good with most of the patients achieving full vestibular recovery with no subsequent incidents of vertigo.
