Abstract Benzodiazepini su jedni od najčešće propisivanih lijekova na svijetu. Iako su lijekovi iz skupine anksiolitika cjenovno veoma prihvatljivi, posljedice koje se javljaju zbog dugotrajnog uzimanja značajno opterećuju zdravstveni sustav. U skladu s time opravdana je potreba za procjenom postojećeg stanja radi daljnje racionalizacije liječničkog propisivanja lijekova.
U Republici Hrvatskoj uočen je uzlazni trend propisivanja benzodiazepina, udio benzodiazepinskih recepata iznosio je 8,67 % ukupne potrošnje lijekova u 2015., odnosno 8,76 % u ukupnoj potrošnji lijekova u 2016. godini. Dijagnoze pod kojima su najčešće propisivani benzodiazepini bile su anksiozni poremećaj, depresivna epizoda i ostali neurotski poremećaji. Gotovo svaki četvrti osiguranik u Republici Hrvatskoj koristio je benzodiazepine u 2015. (19,05 %) i 2016. godini (19,29 %), pri čemu starije dobne skupine češće nego mlađe, a najveća zastupljenost korištenja benzodiazepina bila je u dobnoj skupini 60 – 69 godina. Među bolesnicima starijim od 60 godina najčešće propisivan anksiolitik u 2015. i 2016. godini bio je diazepam, a slijede alprazolam i oksazepam, za oba spola. Ovo istraživanje naglašava, na nacionalnoj razini, da se velikom postotku bolesnika u dobi od 60 godina i starijih obično produljeno propisuju benzodiazepini, unatoč tomu što je prilikom duljeg korištenja njihova klinička korisnost upitna te potencijalno povezana s povećanim rizikom od pojave štetnih učinaka. Među starijom dobnom skupinom (60 godina i stariji), benzodiazepini su najčešće propisani u Osječko-baranjskoj, a zatim u Koprivničko-križevačkoj i Vukovarsko-srijemskoj županiji, dok su županije s najmanjim brojem ostvarenih recepata po bolesniku bile Ličko-senjska, Istarska i Zadarska županija. Podaci predstavljeni u ovom istraživanju upućuju na to da prilikom propisivanja benzodiazepina treba biti oprezan te da prilikom duljeg propisivanja tih lijekova treba uzeti u obzir lijekove iz drugih skupina sa sličnim učinkom ili nefarmakološki način liječenja, posebice u starijoj populaciji koja je izložena većem riziku od razvoja tolerancije, ovisnosti i drugih nuspojava.
Uzimajući u obzir kako je u Republici Hrvatskoj osnovno zdravstveno osiguranje obvezno, neophodna su istraživanja gdje se na transparentan i znanstveno utemeljen način prikazuju podaci o korištenju lijekova s ciljem postizanja racionalne farmakoterapije. Također, farmakoepidemiološkom procjenom stanja ostvaren je prvi korak u definiranju strategije za racionalizaciju propisivanja benzodiazepina na razini izabranog liječnika obiteljske medicine, radi ostvarivanja farmakoekonomske koristi za društvo, ne samo nacionalno, već i globalno.
Abstract (english) Benzodiazepines are one of the most frequently prescribed drugs in the world. Although anxiolytics are part of a cost-effective medicine group, long-term administration significantly affects the health system. Due to this, there is a need to evaluate and potentially rationalize physician's prescription habits.
In the Republic of Croatia, the number of utilized benzodiazepines showed an increasing pattern from 8.67 % in 2015 to 8.76 % in 2016. Benzodiazepines were most commonly prescribed for diagnoses of anxiety disorder, depressive episode and other neurotic disorders. Almost every fourth insured person in the Republic of Croatia utilized benzodiazepines in 2015 (19.05 %) and 2016 (19.29 %), with older age groups more frequent than younger population, while the highest prevalence of benzodiazepine use was noted in the age group 60-69 years. In 2015 and 2016 the most commonly prescribed benzodiazepine among elderly was diazepam, followed by alprazolam and oxazepam, for both sexes. This study highlight, at a national level, that large proportion of patients aged 60 years or older are commonly prescribed with benzodiazepines, even though their prolonged clinical benefit is questionable, and their utilization is related to increased risk of adverse drug effects. Among the older age group (60 years and older), benzodiazepines were most commonly prescribed in Osijek-Baranja, followed by Koprivnica-Križevci and Vukovar-Srijem counties, while the counties with the lowest number of benzodiazepine prescriptions per patient were Lika-Senj, Istria and Zadar counties. The data presented in this study suggests that caution should be taken when prescribing benzodiazepines. Also, it should be taken into consideration when these types of medications are prescribed through a longer period of time to either substitute them with other drugs from groups with similar effects or even decide for a non-pharmacological approach, especially among the older population. This age group has a higher risk of numerous side effects, including tolerance and dependence development. The Republic of Croatia has compulsory health insurance which is the reason why there is an urgent need for up-to-date data regarding drug utilization, presented transparently and scientifically based, in order to obtain rational pharmacotherapy use. In addition, pharmacoepidemiological assessment represents the first step in defining a strategy for improving the rationalization of benzodiazepine prescription among family medicine doctors, with the aim of pharmacoeconomic benefit for society, not only nationally but also globally.
