Abstract

Relativno novonastala pandemija bolesti COVID-19 uzrokovala je posvemašnju zabrinutost za osobe starije dobi, kao i za one osobe koje boluju od kroničnih bolesti kao što su KOPB i astma. Što se tiče COVID-19 i KOPB-a, istraživanja su pokazala da oboljeli od KOPB-a imaju nešto veći rizik za potrebnu hospitalizaciju prilikom infekcije sa SARS-CoV-2 uzročnikom te viši mortalitet i veći rizik za prijam u jedinice intenzivnog liječenja u odnosu na opću populaciju. Trenutni pogled na liječenje KOPB-a u kombinaciji s COVID-19 je taj da nema dovoljno podataka iz istraživanja koji bi upućivali da korištenje uobičajenih lijekova što u terapiji održavanja KOPB-a ili u egzacerbacijama pogoduju lošijim ishodima bolesti. Iako su podaci iz brojnih studija podvojeni, konačno mišljenje je da astma ne predstavlja nezavisni čimbenik rizika za stjecanje infekcije SARS-CoV-2 uzročnikom. Nadalje, osobe s dobro kontroliranom astmom koje istovremeno obole od COVID-19 nemaju veću stopu mortaliteta ni veću učestalost hospitalizacija u odnosu na ostatak populacije. Preporuča se nastavak korištenja osnovnih lijekova u terapiji održavanja astme – inhalacijskih kortikosteroida i za vrijeme pandemije COVID-19 jer dobro kontrolirana astma ne predstavlja rizični čimbenik za COVID-19. Rizičnu skupinu za teži oblik kliničke slike COVID-19 i višu stopu mortaliteta predstavljaju one osobe s loše kontroliranom astmom koje koriste sistemske kortikosteroide zbog liječenja i/ili prevencije egzacerbacija astme.

Abstract (english)

The relatively new COVID-19 pandemic has caused significant concern regarding older people and those who suffer from chronic diseases such as COPD and asthma. As far as COVID-19 and COPD, studies have shown that people suffering from COPD have slightly higher risk for necessary hospitalization while infected with SARS-CoV-2, higher mortality and higher risk for admission in intenstive care units in comparison with general population. Current view of COPD treatment with COVID-19 is that there is not enough information which would refer that use of current medications for COPD maintenance therapy or therapy for COPD exacerbations favor worse disease outcomes. Although the data from numerous studies is indistinctive, the final opinion is that asthma does not represent an independent risk factor for acquiring infection with the SARS-CoV-2. Furthermore, people with well-controlled asthma who simultaneously develop COVID-19 do not have a higher mortality rate or a higher frequency of hospitalizations compared to the rest of the population. It is recommended to continue the use of key medicines in maintenance therapy for asthma - inhaled corticosteroids even during the COVID-19 pandemic, because well-controlled asthma is not a risk factor for COVID-19. People with poorly controlled asthma who use systemic corticosteroids for the treatment and/or prevention of asthma exacerbations represent a risk group that shows higher mortality rate and more often severe form of COVID-19.
